When hooking up to an HDMI analyzer noticed some timings were
off by one. Referring to the hardware technical reference manual
for the lcd controller some of the timing registers use 0 to
represent 1. This patch addresses that issue.

Adding support for max-pixelclock and max-width device tree

TI LCD controller version 2 has an extended eleventh
bit that enables horizontal resolutions greater than
1024 pixels to be specified (upto 2048). This patch
adds support for setting this bit on LCDC V2.

At the larger resolutions, the g200e series sometimes struggles with
maintaining a proper output. Problems like flickering or black bands appearing
on screen can occur. In order to avoid this, limitations regarding resolutions
and bandwidth have been added for the different variations of the g200e series.
This code was ported from the old xorg mga driver.

drm_mode_detailed() is called quite often, therefore when a monitor
that has a detailed timing mode marked DRM_EDID_PT_STEREO or requiring
composite sync, warning messages will clutter up the kernel log.
Like we already do for incorrect hsync/vsync pluse widths, print these
messages only when KMS debugging is enabled.

When dev->driver->master_set() failed ioctl call return 0
but the caller is not the DRM-Master because file_priv->is_master = 0.
Fix that by returning to ioctl caller the driver master_set error code.

When CONFIG_PROVE_LOCKING is not enabled, more tests are
expected to pass unexpectedly, but there no tests that should
start to fail that pass with CONFIG_PROVE_LOCKING enabled.

None of the ww_mutex codepaths should be taken in the 'normal'
mutex calls. The easiest way to verify this is by using the
normal mutex calls, and making sure o.ctx is unmodified.

Injects EDEADLK conditions at pseudo-random interval, with
exponential backoff up to UINT_MAX (to ensure that every lock
operation still completes in a reasonable time).
This way we can test the wound slowpath even for ww mutex users
where contention is never expected, and the ww deadlock
avoidance algorithm is only needed for correctness against
malicious userspace. An example would be protecting kernel
modesetting properties, which thanks to single-threaded X isn't
really expected to contend, ever.
I've looked into using the CONFIG_FAULT_INJECTION
infrastructure, but decided against it for two reasons:
- EDEADLK handling is mandatory for ww mutex users and should
never affect the outcome of a syscall. This is in contrast to -ENOMEM
injection. So fine configurability isn't required.
- The fault injection framework only allows to set a simple
probability for failure. Now the probability that a ww mutex acquire
stage with N locks will never complete (due to too many injected
EDEADLK backoffs) is zero. But the expected number of ww_mutex_lock
operations for the completely uncontended case would be O(exp(N)).
The per-acuiqire ctx exponential backoff solution choosen here only
results in O(log N) overhead due to injection and so O(log N * N)
lock operations. This way we can fail with high probability (and so
have good test coverage even for fancy backoff and lock acquisition
paths) without running into patalogical cases.
Note that EDEADLK will only ever be injected when we managed to
acquire the lock. This prevents any behaviour changes for users
which rely on the EALREADY semantics.

Wound/wait mutexes are used when other multiple lock
acquisitions of a similar type can be done in an arbitrary
order. The deadlock handling used here is called wait/wound in
the RDBMS literature: The older tasks waits until it can acquire
the contended lock. The younger tasks needs to back off and drop
all the locks it is currently holding, i.e. the younger task is
wounded.

This will allow me to call functions that have multiple
arguments if fastpath fails. This is required to support ticket
mutexes, because they need to be able to pass an extra argument
to the fail function.
Originally I duplicated the functions, by adding
__mutex_fastpath_lock_retval_arg. This ended up being just a
duplication of the existing function, so a way to test if
fastpath was called ended up being better.
This also cleaned up the reservation mutex patch some by being
able to call an atomic_set instead of atomic_xchg, and making it
easier to detect if the wrong unlock function was previously
used.

This broke the WWPN and LUN sysfs attributes for s390 reipl and dump
on panic.
Example:

So fix this and use format strings that work also with the
new sscanf implementation:

With git commit 996b4a7d "s390/mem_detect: remove artificial kdump
memory types" the memory detection code got simplified.
As a side effect the array that describes memory chunks may now
contain empty (zeroed) entries.
All call sites can handle this except for
drivers/s390/char/zcore.c::zcore_memmap_open
which has a really odd user space interface. The easiest fix is to
change the memory hole handling code, so that no empty entries exist
before the last valid entry is reached.

Up to now the PCIe link status on Freescale PCIe controllers was only
checked once at boot time. So hotplug did not work. With this patch the
link status is checked on every config read. PCIe devices not present at
boot time are found after doing 'echo 1 >/sys/bus/pci/rescan'.

Changing size of a file on server and local update (fuse_write_update_size)
should be always protected by inode->i_mutex. Otherwise a race like this is
possible:
1. Process 'A' calls fallocate(2) to extend file (~FALLOC_FL_KEEP_SIZE).
fuse_file_fallocate() sends FUSE_FALLOCATE request to the server.
2. Process 'B' calls ftruncate(2) shrinking the file. fuse_do_setattr()
sends shrinking FUSE_SETATTR request to the server and updates local i_size
by i_size_write(inode, outarg.attr.size).
3. Process 'A' resumes execution of fuse_file_fallocate() and calls
fuse_write_update_size(inode, offset + length). But 'offset + length' was
obsoleted by ftruncate from previous step.
Changed in v2 (thanks Brian and Anand for suggestions):
- made relation between mutex_lock() and fuse_set_nowrite(inode) more
explicit and clear.

If the device is already in a runtime PM enabled state
pm_runtime_get_sync() will return 1, so a test for negative
value should be used to check for errors.
Without this patch there are seen errors like:

Likely because the driver uses synchronous API to runtime enable
the device and asynchronous one to disable it.

pxa2xx_spi_map_dma_buffer() gets called in tasklet context so we can't
sleep when we allocate a new sg table. Use GFP_ATOMIC here instead.

Added MAP_TRIGGERS_TO_BUTTONS for Mad Catz Street Fighter IV FightPad
device. This controller model was already supported by the xpad
driver, but none of the buttons work correctly without this change.
Tested on kernel version 3.9.5.

The command and status register in the driver were swapped with
respect to the order specified in the datasheet (CY8CTMA140).
Confirmed with Cypress that the order in the datasheet is correct.

memcpy param is wrong because of offset in bl_cmd, this may corrupt the

Commit c011470 (irqchip: gic: Perform the gic_secondary_init() call via
CPU notifier) moves gic_secondary_init() that used to be called in
.smp_secondary_init hook into a notifier call. But it changes the
system behavior a little bit. Before the commit, gic_cpu_init()
is called not only when kernel brings up the secondary cores but also
when system resuming procedure hot-plugs the cores back to kernel.
While after the commit, the function will not be called in the latter
case, where the 'action' will not be CPU_STARTING but
CPU_STARTING_FROZEN. This behavior difference at least causes the
following suspend/resume regression on imx6q.

fw_priv->buf is accessed in both request_firmware_load() and
writing to sysfs file of 'loading' context, but not protected
by 'fw_lock' entirely. The patch makes sure that access on
'fw_priv->buf' is protected by the lock.
So fixes the double abort problem reported by nirinA raseliarison:
