Commit e66eed651fd1 ("list: remove prefetching from regular list
iterators") removed the include of prefetch.h from list.h, which
uncovered several cases that had apparently relied on that rather
obscure header file dependency.
So this fixes things up a bit, using
grep -L linux/prefetch.h $(git grep -l '[^a-z_]prefetchw*(' -- '*.[ch]')
grep -L 'prefetchw*(' $(git grep -l 'linux/prefetch.h' -- '*.[ch]')
to guide us in finding files that either need <linux/prefetch.h>
inclusion, or have it despite not needing it.
There are more of them around (mostly network drivers), but this gets
many core ones.

There is a macro called dev_info that prints struct device specific
information. Having variables with the same name can be confusing and
prevents conversion of the macro to a function.
Rename the existing dev_info variables to something else in preparation
to converting the dev_info macro to a function.

It's possible that SBA IOMMU might fail to find I/O space under heavy
I/Os. SBA IOMMU panics on allocation failure but it shouldn't; drivers
can handle the failure. The majority of other IOMMU drivers don't panic
on allocation failure.
This patch fixes SBA IOMMU path to handle allocation failure properly.

Completed a major update for the acpi_get_object_info external interface.
Changes include:
- Support for variable, unlimited length HID, UID, and CID strings
- Support Processor objects the same as Devices (HID,UID,CID,ADR,STA, etc.)
- Call the _SxW power methods on behalf of a device object
- Determine if a device is a PCI root bridge
- Change the ACPI_BUFFER parameter to ACPI_DEVICE_INFO.
These changes will require an update to all callers of this interface.
See the ACPICA Programmer Reference for details.

It is generally agreed that it would be beneficial for u64 to be an
unsigned long long on all architectures. ia64 (in common with several
other 64-bit architectures) currently uses unsigned long. Migrating
piecemeal is too painful; this giant patch fixes all compilation warnings
and errors that come as a result of switching to use int-ll64.h.
Note that userspace will still see __u64 defined as unsigned long. This
is important as it affects C++ name mangling.

u64 for start/end rather than unsigned long]

Callers of alloc_pages_node() can optionally specify -1 as a node to mean
"allocate from the current node". However, a number of the callers in
fast paths know for a fact their node is valid. To avoid a comparison and
branch, this patch adds alloc_pages_exact_node() that only checks the nid
with VM_BUG_ON(). Callers that know their node is valid are then
converted.

Move DMA-mapping.txt to Documentation/PCI/.
DMA-mapping.txt was supposed to be moved from Documentation/ to
Documentation/PCI/. The 00-INDEX files in those two directories
were updated, along with a few other text files, but the file
itself somehow escaped being moved, so move it and update more
text files and source files with its new location.

This converts X86 and IA64 to use include/linux/dma-mapping.h.
It's a bit large but pretty boring. The major change for X86 is
converting 'int dir' to 'enum dma_data_direction dir' in DMA mapping
operations. The major changes for IA64 is using map_page and
unmap_page instead of map_single and unmap_single.

This patch introduces a global pointer, dma_ops, which points to an
appropriate dma_mapping_ops that the kernel should use. This is a
common way to handle multiple dma_mapping_ops (X86, POWER, and SPARC).
dma_ops is set in platform_dma_init. We also set it by hand where
machvec_init is callev via subsys_initcall.
- IA64_DIG_VTD uses vtd_dma_ops.
- IA64_HP_ZX1 uses sba_dma_ops.
- IA64_HP_ZX1_SWIOTLB uses hwsw_dma_ops.
- IA64_SGI_SN2 uses sn_dma_ops.
- The rest use swiotlb_dma_ops.

Add per-device dma_mapping_ops support for CONFIG_X86_64 as POWER
architecture does:
This enables us to cleanly fix the Calgary IOMMU issue that some devices
are not behind the IOMMU (http://lkml.org/lkml/2008/5/8/423).
I think that per-device dma_mapping_ops support would be also helpful for
KVM people to support PCI passthrough but Andi thinks that this makes it
difficult to support the PCI passthrough (see the above thread). So I
CC'ed this to KVM camp. Comments are appreciated.
A pointer to dma_mapping_ops to struct dev_archdata is added. If the
pointer is non NULL, DMA operations in asm/dma-mapping.h use it. If it's
NULL, the system-wide dma_ops pointer is used as before.
If it's useful for KVM people, I plan to implement a mechanism to register
a hook called when a new pci (or dma capable) device is created (it works
with hot plugging). It enables IOMMUs to set up an appropriate
dma_mapping_ops per device.
The major obstacle is that dma_mapping_error doesn't take a pointer to the
device unlike other DMA operations. So x86 can't have dma_mapping_ops per
device. Note all the POWER IOMMUs use the same dma_mapping_error function
so this is not a problem for POWER but x86 IOMMUs use different
dma_mapping_error functions.
The first patch adds the device argument to dma_mapping_error. The patch
is trivial but large since it touches lots of drivers and dma-mapping.h in
all the architecture.
This patch:
dma_mapping_error() doesn't take a pointer to the device unlike other DMA
operations. So we can't have dma_mapping_ops per device.
Note that POWER already has dma_mapping_ops per device but all the POWER
IOMMUs use the same dma_mapping_error function. x86 IOMMUs use device
argument.

Change all ia64 machvecs to use the new dma_*map*_attrs() interfaces.
Implement the old dma_*map_*() interfaces in terms of the corresponding new
interfaces. For ia64/sn, make use of one dma attribute,
DMA_ATTR_WRITE_BARRIER. Introduce swiotlb_*map*_attrs() functions.

IA64's IOMMU implementation allocates memory areas spanning LLD's segment
boundary limit. It forces low level drivers to have a workaround to adjust
scatter lists that the IOMMU builds.
We are in the process of making all the IOMMUs respect the segment boundary
limits to remove such work around in LLDs. This patch is for IA64's IOMMU.

This driver for HPQ5001 devices installs a global ACPI OpRegion handler.
AML methods can use this OpRegion to call native firmware entry points.
ACPI does not define a mechanism for AML methods to call native firmware
interfaces such as PAL or SAL. This OpRegion handler adds such a mechanism.
After the handler is installed, an AML method can call native firmware by
storing the arguments and firmware entry point to specific offsets in the
OpRegion. When AML reads the "return value" offset from the OpRegion, this
handler loads up the arguments, makes the firmware call, and returns the
result.

On HP zx1 machines, the 'machvec=dig' parameter is needed for the
kdump kernel to avoid problems with the HP sba iommu. The problem
is that during the boot of the kdump kernel, the iommu is re-initialized,
so in-flight DMA from improperly shutdown drivers causes an IOTLB
miss which leads to an MCA. With kdump, the idea is to get into the
kdump kernel with as little code as we can, so shutting down drivers
properly is not an option.
The workaround is to add 'machvec=dig' to the kdump kernel boot
parameters. This makes the kdump kernel avoid using the sba iommu
altogether, leaving the IOTLB intact. Any ongoing DMA falls
harmlessly outside the kdump kernel. After the kdump kernel reboots,
all devices will have been shutdown properly and DMA stopped.
This patch pushes that functionality into the sba iommu
initialization code, so that users won't have to find the obscure
documentation telling them about 'machvec=dig'.
This patch only affects HP platforms. It still includes one
extern declaration in the file, because no applicable header file
exists.

Many struct file_operations in the kernel can be "const". Marking them const
moves these to the .rodata section, which avoids false sharing with potential
dirty data. In addition it'll catch accidental writes at compile time to
these shared resources.

Machine vector selection has always been a bit of a hack given how
early in system boot it needs to be done. Services like ACPI namespace
are not available and there are non-trivial problems to moving them to
early boot. However, there's no reason we can't change to a different
machvec later in boot when the services we need are available. By
adding a entry point for later initialization of the swiotlb, we can add
an error path for the hpzx1 machevec initialization and fall back to the
DIG machine vector if IOMMU hardware isn't found in the system. Since
ia64 uses 4GB for zone DMA (no ISA support), it's trivial to allocate a
contiguous range from the slab for bounce buffer usage.

No functional change, just identify the device nicely:

We used to create fake PCI devices for these chips, but we no longer do that.
So I don't think there's any reason to touch pci_ids.h now.
