This helps to make following hash only pte bits easier.
We have kept _PAGE_CHG_MASK, _HPAGE_CHG_MASK and _PAGE_PROT_BITS as it
is in this patch eventhough they use hash specific bits. Using them in
radix as it is should be ok, because with radix we expect those bit
positions to be zero.
Only renames in this patch, no change in functionality.

We need to properly identify whether a hugepage is an explicit or
a transparent hugepage in follow_huge_addr(). We used to depend
on hugepage shift argument to do that. But in some case that can
result in wrong results. For ex:
On finding a transparent hugepage we set hugepage shift to PMD_SHIFT.
But we can end up clearing the thp pte, via pmdp_huge_get_and_clear.
We do prevent reusing the pfn page via the usage of
kick_all_cpus_sync(). But that happens after we updated the pte to 0.
Hence in follow_huge_addr() we can find hugepage shift set, but transparent
huge page check fail for a thp pte.
NOTE: We fixed a variant of this race against thp split in commit
691e95fd7396905a38d98919e9c150dbc3ea21a3
("powerpc/mm/thp: Make page table walk safe against thp split/collapse")
Without this patch, we may hit the BUG_ON(flags & FOLL_GET) in
follow_page_mask occasionally.
In the long term, we may want to switch ppc64 64k page size config to
enable CONFIG_ARCH_WANT_GENERAL_HUGETLB

If we changed base page size of the segment, either via sub_page_protect
or via remap_4k_pfn, we do a demote_segment which doesn't flush the hash
table entries. We do a lazy hash page table flush for all mapped pages
in the demoted segment. This happens when we handle hash page fault for
these pages.
We use _PAGE_COMBO bit along with _PAGE_HASHPTE to indicate whether a
pte is backed by 4K hash pte. If we find _PAGE_COMBO not set on the pte,
that implies that we could possibly have older 64K hash pte entries in
the hash page table and we need to invalidate those entries.
Use _PAGE_COMBO to determine the page size with which we should
invalidate the hash table entries on unmap.

None of these files are actually using any __init type directives
and hence don't need to include <linux/init.h>. Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.
The one instance where we add an include for init.h covers off
a case where that file was implicitly getting it from another
header which itself didn't need it.

Pull powerpc updates from Ben Herrenschmidt:
"This is the powerpc changes for the 3.11 merge window. In addition to
the usual bug fixes and small updates, the main highlights are:
- Support for transparent huge pages by Aneesh Kumar for 64-bit
server processors. This allows the use of 16M pages as transparent
huge pages on kernels compiled with a 64K base page size.
- Base VFIO support for KVM on power by Alexey Kardashevskiy
- Wiring up of our nvram to the pstore infrastructure, including
putting compressed oopses in there by Aruna Balakrishnaiah
- Move, rework and improve our "EEH" (basically PCI error handling
and recovery) infrastructure. It is no longer specific to pseries
but is now usable by the new "powernv" platform as well (no
hypervisor) by Gavin Shan.
- I fixed some bugs in our math-emu instruction decoding and made it
usable to emulate some optional FP instructions on processors with
hard FP that lack them (such as fsqrt on Freescale embedded
processors).
- Support for Power8 "Event Based Branch" facility by Michael
Ellerman. This facility allows what is basically "userspace
interrupts" for performance monitor events.
- A bunch of Transactional Memory vs. Signals bug fixes and HW
breakpoint/watchpoint fixes by Michael Neuling.

We now have pmd entries covering 16MB range and the PMD table double its original size.
We use the second half of the PMD table to deposit the pgtable (PTE page).
The depoisted PTE page is further used to track the HPTE information. The information
include [ secondary group | 3 bit hidx | valid ]. We use one byte per each HPTE entry.
With 16MB hugepage and 64K HPTE we need 256 entries and with 4K HPTE we need
4096 entries. Both will fit in a 4K PTE page. On hugepage invalidate we need to walk
the PTE page and invalidate all valid HPTEs.
This patch implements necessary arch specific functions for THP support and also
hugepage invalidate logic. These PMD related functions are intentionally kept
similar to their PTE counter-part.

Ever since commit 45f035ab9b8f ("CONFIG_HOTPLUG should be always on"),
it has been basically impossible to build a kernel with CONFIG_HOTPLUG
turned off. Remove all the remaining references to it.

This patch change the kernel VSID range so that we limit VSID_BITS to 37.
This enables us to support 64TB with 65 bit VA (37+28). Without this patch
we have boot hangs on platforms that only support 65 bit VA.
With this patch we now have proto vsid generated as below:
We first generate a 37-bit "proto-VSID". Proto-VSIDs are generated
from mmu context id and effective segment id of the address.
For user processes max context id is limited to ((1ul << 19) - 5)
for kernel space, we use the top 4 context ids to map address as below

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

This patch convert different functions to take virtual page number
instead of virtual address. Virtual page number is virtual address
shifted right by VPN_SHIFT (12) bits. This enable us to have an
address range of upto 76 bits.

In case other architectures require RCU freed page-tables to implement
gup_fast() and software filled hashes and similar things, provide the
means to do so by moving the logic into generic code.

Fix up powerpc to the new mmu_gather stuff.
PPC has an extra batching queue to RCU free the actual pagetable
allocations, use the ARCH extentions for that for now.
For the ppc64_tlb_batch, which tracks the vaddrs to unhash from the
hardware hash-table, keep using per-cpu arrays but flush on context switch
and use a TLF bit to track the lazy_mmu state.

hpte_need_flush() might be called outside of a preempt section
when manipulating the kernel page tables, so we need to use the
appopriate variants of per-cpu variable accesses. There should
be no risk of being in the middle of a batch and a context
switch will flush any pending batch.

Commit f71dc176aa06359681c30ba6877ffccab6fba3a6 'Make
hpte_need_flush() correctly mask for multiple page sizes' introduced
bug, which is triggered when a kernel with a 64k base page size is run
on a system whose hardware does not 64k hash PTEs. In this case, we
emulate 64k pages with multiple 4k hash PTEs, however in
hpte_need_flush() we incorrectly only mask the hardware page size from
the address, instead of the logical page size. This causes things to
go wrong when we later attempt to iterate through the hardware
subpages of the logical page.
This patch corrects the error. It has been tested on pSeries bare
metal by Michael Neuling.

Currently, hpte_need_flush() only correctly flushes the given address
for normal pages. Callers for hugepages are required to mask the
address themselves.
But hpte_need_flush() already looks up the page sizes for its own
reasons, so this is a rather silly imposition on the callers. This
patch alters it to mask based on the pagesize it has looked up itself,
and removes the awkward masking code in the hugepage caller.

The definition for the global structure mmu_gathers, used by generic code,
is currently defined in multiple places not including anything used by
64-bit Book3E. This changes it by moving to one place common to all
processors.

That patch used to just add a hook to page table flushing but
pulling that string brought out a whole bunch of issues, so it
now does that and more:
- We now make the RCU batching of page freeing SMP only, as I
believe it was intended initially. We make a few more things compile
to nothing on !CONFIG_SMP
- Some macros are turned into functions, though that forced me to
out of line a few stuffs due to unsolvable include depenencies,
however it's probably better that way anyway, it's not -that-
critical code path.
- 32-bit didn't call pte_free_finish() on tlb_flush() which means
that it wouldn't push out the batch to RCU for delayed freeing when
a bunch of page tables have been freed, they would just stay in there
until the batch gets full.
64-bit BookE will use that hook to maintain the virtually linear
page tables or the indirect entries in the TLB when using the
HW loader.

Makes code futureproof against the impending change to mm->cpu_vm_mask.
It's also a chance to use the new cpumask_ ops which take a pointer
(the older ones are deprecated, but there's no hurry for arch code).

This renames the files to clarify the fact that they are used by
the hash based family of CPUs (the 603 being an exception in that
family but is still handled by that code).
This paves the way for the new tlb_nohash.c coming via a subsequent
commit.
