Glen Turner reported that writing LFCR rather than the more
traditional CRLF causes issues with some terminals.
Since this afflicts many serial drivers, extract the common code to a
library function (uart_console_write) and arrange for each driver to
supply a "putchar" function.
but early_printk is left out.

The console subsystem already has an idea of a boot console, using the
CON_BOOT flag. The implementation has some flaws though. The major
problem is that presence of a boot console makes register_console() ignore
any other console devices (unless explicitly specified on the kernel
command line).
This patch fixes the console selection code to *not* consider a boot
console a full-featured one, so the first non-boot console registering will
become the default console instead. This way the unregister call for the
boot console in the register_console() function actually triggers and the
handover from the boot console to the real console device works smoothly.
Added a printk for the handover, so you know which console device the
output goes to when the boot console stops printing messages.
The disable_early_printk() call is obsolete with that patch, explicitly
disabling the early console isn't needed any more as it works automagically
with that patch.
I've walked through the tree, dropped all disable_early_printk() instances
found below arch/ and tagged the consoles with CON_BOOT if needed. The
code is tested on x86, sh (thanks to Paul) and mips (thanks to Ralf).
Changes to last version: Rediffed against -rc3, adapted to mips cleanups by
Ralf, fixed "udbg-immortal" cmd line arg on powerpc.

Commit 2c8c0e6b8d7700a990da8d24eff767f9ca223b96 ("[PATCH] Convert x86-64
to early param") broke the earlyprintk=...,keep feature.
This restores that functionality. Tested on x86_64. Must-have for
v2.6.19, no risk.

Instead of hackish manual parsing
Requires earlier i386 patchkit, but also fixes i386 early_printk again.
I removed some obsolete really early parameters which didn't do anything useful.
Also made a few parameters that needed it early (mostly oops printing setup)
Also removed one panic check that wasn't visible without
early console anyways (the early console is now initialized after that
panic)
This cleans up a lot of code.

screen_info.h doesn't have anything to do with the tty layer and shouldn't be
included by tty.h. This patches removes the include and modifies all users to
directly include screen_info.h. struct screen_info is mainly used to
communicate with the console drivers in drivers/video/console. Note that this
patch touches every arch and I have no way of testing it. If there is a
mistake the worst thing that will happen is a compile error.

The boot cmdline is parsed in parse_early_param() and
parse_args(,unknown_bootoption).
And __setup() is used in obsolete_checksetup().
start_kernel()
-> parse_args()
-> unknown_bootoption()
-> obsolete_checksetup()
If __setup()'s callback (->setup_func()) returns 1 in
obsolete_checksetup(), obsolete_checksetup() thinks a parameter was
handled.
If ->setup_func() returns 0, obsolete_checksetup() tries other
->setup_func(). If all ->setup_func() that matched a parameter returns 0,
a parameter is seted to argv_init[].
Then, when runing /sbin/init or init=app, argv_init[] is passed to the app.
If the app doesn't ignore those arguments, it will warning and exit.
This patch fixes a wrong usage of it, however fixes obvious one only.

The history is that -mm kernels do not work for me for a few months
already. The things started from crashing somewhere after starting init,
and for the last month - no boot at all, just "Uncompressing... OK,
booting kernel", and silence. Early console didn't work too. With the
latest releases this degraded into an infinite stream of the "Unknown
interrupt or fault" messages. So today my patience ran out and I started
to think how can I collect at least some info for the bug-report. Attached
is the patch that allows to gather some valueable debug info on the problem
by making an early console more useable. I can't properly test the patch,
as the kernel still doesn't boot, so I'll explain it in details in a hope
someone else can justify the intrusive changes.
arch_hooks.h: added prototypes for setup_early_printk() and early_printk().
setup.c: killed wrong setup_early_printk() prototype. Moved
setup_early_printk() a bit earlier, as it was not "early enough" to cover
the bug I was fighting with.
early_printk.c: made it to start printing from the bottom of the screen,
otherwise the messages interfere with the ones of the boot-loader, so you
can't read them.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
