The dev_uevent() in struct class should not be modifying the device that
is passed into it, so mark it as a const * and propagate the function
signature changes out into all relevant subsystems that use this
callback.

Delete/fixup few includes in anticipation of global -isystem compile
option removal.
Note: crypto/aegis128-neon-inner.c keeps <stddef.h> due to redefinition
of uintptr_t error (one definition comes from <stddef.h>, another from
<linux/types.h>).

'struct timeval' uses 32-bit representation for seconds which will
overflow in year 2038 and beyond. mISDN/clock.c needs to compute and
store elapsed time in intervals of 125 microseconds. This patch replaces
the usage of 'struct timeval' with 64-bit ktime_t which is y2038 safe.
The patch also replaces do_gettimeofday() (wall-clock time) with
ktime_get() (monotonic time) since we only care about elapsed time here.

If the FIFO of the card is small, many short messages are queued up to
the upper layers and the userspace. This change allows the applications
to set a minimum datalen they want from the drivers.
Create a common control function to avoid code duplication in each
driver.

With multiple cards is hard to figure out which port caused trouble
int the layer2 routines (e.g. got a timeout).

To be full preemptiv safe, we cannot handle a L2 timeout in the timer
context itself, we should do all actions via the D-channel thread.

MPH_INFORMATION provides full D- and B-Channel status overview
- new layer1 primitive: MPF_INFORMATON_REQ
- layer1 replies with MPH_INFORMATION_IND containing
- dch->[state,Flags,nrbchan]

New prim PH_DATA_E_IND.
- all E-ch frames are indicated by recv_Echannel(), which pushes E-Channel
frames into dch's rqueue
- if dchannel is opened with channel nr 0, no E-Channel logging
is requested
- if dchannel is opened with channel nr 1, E-Channel logging
is requested. if layer1 does not support that, -EINVAL
in return is appropriate

This prevents underrun of fifo when filled and in case of an underrun it
prevents subsequent underruns due to jitter.
Improve dsp, so buffers are kept filled with a certain delay, so moderate
jitter will not cause underrun all the time -> the audio quality is highly
improved. tones are not interrupted by gaps anymore, except when CPU is
stalling or in high load.
