StdLib/LibC/gdtoa: Initialize variables before use, update the Kmax definition.
gdtoaimp.h: update the Kmax definition. strtodg.c: initialize variables before use. general: Remove conditional sections for VAX and IBM mainframe. Remove conditional sections for pre-ANSI (K&R) function prototypes.
