Depending on the number of online CPUs in the original kernel, it is
likely for CPU #0 to be offline in a kdump kernel. The associated IRQs
in the affinity mappings provided by irq_create_affinity_masks() are
thus not started by irq_startup(), as per-design with managed IRQs.
This can be a problem with multi-queue block devices driven by blk-mq :
such a non-started IRQ is very likely paired with the single queue
enforced by blk-mq during kdump (see blk_mq_alloc_tag_set()). This
causes the device to remain silent and likely hangs the guest at
some point.
This is a regression caused by commit 9ea69a55b3b9 ("powerpc/pseries:
Pass MSI affinity to irq_create_mapping()"). Note that this only happens
with the XIVE interrupt controller because XICS has a workaround to bypass
affinity, which is activated during kdump with the "noirqdistrib" kernel
parameter.
The issue comes from a combination of factors:
- discrepancy between the number of queues detected by the multi-queue
block driver, that was used to create the MSI vectors, and the single
queue mode enforced later on by blk-mq because of kdump (i.e. keeping
all queues fixes the issue)
- CPU#0 offline (i.e. kdump always succeed with CPU#0)
Given that I couldn't reproduce on x86, which seems to always have CPU#0
online even during kdump, I'm not sure where this should be fixed. Hence
going for another approach : fine-grained affinity is for performance
and we don't really care about that during kdump. Simply revert to the
previous working behavior of ignoring affinity masks in this case only.

dlpar_configure_connector() has two problems in its handling of
ibm,configure-connector's return status:
1. When the status is -2 (busy, call again), we call
ibm,configure-connector again immediately without checking whether
to schedule, which can result in monopolizing the CPU.
2. Extended delay status (9900..9905) goes completely unhandled,
causing the configuration to unnecessarily terminate.
Fix both of these issues by using rtas_busy_delay().

It's very easy to crash the kernel right now by simply trying to
enable memtrace concurrently, hammering on the "enable" interface
loop.sh:

We currently leak kernel memory to user space, because memory
offlining doesn't do any implicit clearing of memory and we are
missing explicit clearing of memory.
Let's keep it simple and clear pages before removing the linear
mapping.
Reproduced in QEMU/TCG with 10 GiB of main memory:

We execute certain NPU2 setup code (such as mapping an LPID to a device
in NPU2) unconditionally if an Nvlink bridge is detected. However this
cannot succeed on POWER8NVL machines and errors appear in dmesg. This is
harmless as skiboot returns an error and the only place we check it is
vfio-pci but that code does not get called on P8+ either.
This adds a check if pnv_npu2_xxx helpers are called on a machine with
NPU2 which initializes pnv_phb::npu in pnv_npu2_init();
pnv_phb::npu==NULL on POWER8/NVL (Naples).
While at this, fix NULL derefencing in pnv_npu_peers_take_ownership/
pnv_npu_peers_release_ownership which occurs when GPUs on mentioned P8s
cause EEH which happens if "vfio-pci" disables devices using
the D3 power state; the vfio-pci's disable_idle_d3 module parameter
controls this and must be set on Naples. The EEH handling clears
the entire pnv_ioda_pe struct in pnv_ioda_free_pe() hence
the NULL derefencing. We cannot recover from that but at least we stop
crashing.

When SMC1 is relocated and early debug is selected, the
board hangs is ppc_md.setup_arch(). This is because ones
the microcode has been loaded and SMC1 relocated, early
debug writes in the weed.
To allow smooth continuation, the SMC1 parameter RAM set up
by the bootloader have to be copied into the new location.

There are three ways pseries_suspend_begin() can be reached:
1. When "mem" is written to /sys/power/state:
kobj_attr_store()
-> state_store()
-> pm_suspend()
-> suspend_devices_and_enter()
-> pseries_suspend_begin()
This never works because there is no way to supply a valid stream id
using this interface, and H_VASI_STATE is called with a stream id of
zero. So this call path is useless at best.
2. When a stream id is written to /sys/devices/system/power/hibernate.
pseries_suspend_begin() is polled directly from store_hibernate()
until the stream is in the "Suspending" state (i.e. the platform is
ready for the OS to suspend execution):
dev_attr_store()
-> store_hibernate()
-> pseries_suspend_begin()
3. When a stream id is written to /sys/devices/system/power/hibernate
(continued). After #2, pseries_suspend_begin() is called once again
from the pm core:
dev_attr_store()
-> store_hibernate()
-> pm_suspend()
-> suspend_devices_and_enter()
-> pseries_suspend_begin()
This is redundant because the VASI suspend state is already known to
be Suspending.
The begin() callback of platform_suspend_ops is optional, so we can
simply remove that assignment with no loss of function.

With virtio multiqueue, normally each queue IRQ is mapped to a CPU.
Commit 0d9f0a52c8b9f ("virtio_scsi: use virtio IRQ affinity") exposed
an existing shortcoming of the arch code by moving virtio_scsi to
the automatic IRQ affinity assignment.
The affinity is correctly computed in msi_desc but this is not applied
to the system IRQs.
It appears the affinity is correctly passed to rtas_setup_msi_irqs() but
lost at this point and never passed to irq_domain_alloc_descs()
(see commit 06ee6d571f0e ("genirq: Add affinity hint to irq allocation"))
because irq_create_mapping() doesn't take an affinity parameter.
Use the new irq_create_mapping_affinity() function, which allows to forward
the affinity setting from rtas_setup_msi_irqs() to irq_domain_alloc_descs().
With this change, the virtqueues are correctly dispatched between the CPUs
on pseries.

Add a test modelled on the RFI flush test which counts the number
of L1D misses doing a simple syscall with the entry flush on and off.
For simplicity of backporting, this test duplicates a lot of code from
the upstream rfi_flush. This is cleaned up upstream, but we don't clean
it up here because it would involve bringing in even more commits.

IBM Power9 processors can speculatively operate on data in the L1 cache
before it has been completely validated, via a way-prediction mechanism. It
is not possible for an attacker to determine the contents of impermissible
memory using this method, since these systems implement a combination of
hardware and software security measures to prevent scenarios where
protected data could be leaked.
However these measures don't address the scenario where an attacker induces
the operating system to speculatively execute instructions using data that
the attacker controls. This can be used for example to speculatively bypass
"kernel user access prevention" techniques, as discovered by Anthony
Steinhauser of Google's Safeside Project. This is not an attack by itself,
but there is a possibility it could be used in conjunction with
side-channels or other weaknesses in the privileged code to construct an
attack.
This issue can be mitigated by flushing the L1 cache between privilege
boundaries of concern. This patch flushes the L1 cache after user accesses.
This is part of the fix for CVE-2020-4788.

Every error log reported by OPAL is exported to userspace through a
sysfs interface and notified using kobject_uevent(). The userspace
daemon (opal_errd) then reads the error log and acknowledges the error
log is saved safely to disk. Once acknowledged the kernel removes the
respective sysfs file entry causing respective resources to be
released including kobject.
However it's possible the userspace daemon may already be scanning
elog entries when a new sysfs elog entry is created by the kernel.
User daemon may read this new entry and ack it even before kernel can
notify userspace about it through kobject_uevent() call. If that
happens then we have a potential race between
elog_ack_store->kobject_put() and kobject_uevent which can lead to
use-after-free of a kernfs object resulting in a kernel crash. eg:

This patch fixes this race by protecting the sysfs file
creation/notification by holding a reference count on kobject until we
safely send kobject_uevent().
The function create_elog_obj() returns the elog object which if used
by caller function will end up in use-after-free problem again.
However, the return value of create_elog_obj() function isn't being
used today and there is no need as well. Hence change it to return
void to make this fix complete.

When an UE or memory error exception is encountered the MCE handler
tries to find the pfn using addr_to_pfn() which takes effective
address as an argument, later pfn is used to poison the page where
memory error occurred, recent rework in this area made addr_to_pfn
to run in real mode, which can be fatal as it may try to access
memory outside RMO region.
Have two helper functions to separate things to be done in real mode
and virtual mode without changing any functionality. This also fixes
the following error as the use of addr_to_pfn is now moved to virtual
mode.
Without this change following kernel crash is seen on hitting UE.

Every dump reported by OPAL is exported to userspace through a sysfs
interface and notified using kobject_uevent(). The userspace daemon
(opal_errd) then reads the dump and acknowledges that the dump is
saved safely to disk. Once acknowledged the kernel removes the
respective sysfs file entry causing respective resources to be
released including kobject.
However it's possible the userspace daemon may already be scanning
dump entries when a new sysfs dump entry is created by the kernel.
User daemon may read this new entry and ack it even before kernel can
notify userspace about it through kobject_uevent() call. If that
happens then we have a potential race between
dump_ack_store->kobject_put() and kobject_uevent which can lead to
use-after-free of a kernfs object resulting in a kernel crash.
This patch fixes this race by protecting the sysfs file
creation/notification by holding a reference count on kobject until we
safely send kobject_uevent().
The function create_dump_obj() returns the dump object which if used
by caller function will end up in use-after-free problem again.
However, the return value of create_dump_obj() function isn't being
used today and there is no need as well. Hence change it to return
void to make this fix complete.

Resolve this problem by disabling each THRMn comparator when handling

At memory hot-remove time we can retrieve an LMB's nid from its
corresponding memory_block. There is no need to store the nid
in multiple locations.
Note that lmb_to_memblock() uses find_memory_block() to get the
corresponding memory_block. As find_memory_block() runs in sub-linear
time this approach is negligibly slower than what we do at present.
In exchange for this lookup at hot-remove time we no longer need to
call memory_add_physaddr_to_nid() during drmem_init() for each LMB.
On powerpc, memory_add_physaddr_to_nid() is a linear search, so this
spares us an O(n^2) initialization during boot.
On systems with many LMBs that initialization overhead is palpable and
disruptive. For example, on a box with 249854 LMBs we're seeing
drmem_init() take upwards of 30 seconds to complete:

As per PAPR we have to look for both EPOW sensor value and event
modifier to identify the type of event and take appropriate action.
In LoPAPR v1.1 section 10.2.2 includes table 136 "EPOW Action Codes":
SYSTEM_SHUTDOWN 3
The system must be shut down. An EPOW-aware OS logs the EPOW error
log information, then schedules the system to be shut down to begin
after an OS defined delay internal (default is 10 minutes.)
Then in section 10.3.2.2.8 there is table 146 "Platform Event Log
Format, Version 6, EPOW Section", which includes the "EPOW Event
Modifier":
For EPOW sensor value = 3
0x01 = Normal system shutdown with no additional delay
0x02 = Loss of utility power, system is running on UPS/Battery
0x03 = Loss of system critical functions, system should be shutdown
0x04 = Ambient temperature too high
All other values = reserved
We have a user space tool (rtas_errd) on LPAR to monitor for
EPOW_SHUTDOWN_ON_UPS. Once it gets an event it initiates shutdown
after predefined time. It also starts monitoring for any new EPOW
events. If it receives "Power restored" event before predefined time
it will cancel the shutdown. Otherwise after predefined time it will
shutdown the system.
Commit 79872e35469b ("powerpc/pseries: All events of
EPOW_SYSTEM_SHUTDOWN must initiate shutdown") changed our handling of
the "on UPS/Battery" case, to immediately shutdown the system. This
breaks existing setups that rely on the userspace tool to delay
shutdown and let the system run on the UPS.

Partition suspension, used for hibernation and migration, requires
that the OS place all but one of the LPAR's processor threads into one
of two states prior to calling the ibm,suspend-me RTAS function:
* the architected offline state (via RTAS stop-self); or
* the H_JOIN hcall, which does not return until the partition
resumes execution
Using H_CEDE as the offline mode, introduced by
commit 3aa565f53c39 ("powerpc/pseries: Add hooks to put the CPU into
an appropriate offline state"), means that any threads which are
offline from Linux's point of view must be moved to one of those two
states before a partition suspension can proceed.
This was eventually addressed in commit 120496ac2d2d ("powerpc: Bring
all threads online prior to migration/hibernation"), which added code
to temporarily bring up any offline processor threads so they can call
H_JOIN. Conceptually this is fine, but the implementation has had
multiple races with cpu hotplug operations initiated from user
space[1][2][3], the error handling is fragile, and it generates
user-visible cpu hotplug events which is a lot of noise for a platform
feature that's supposed to minimize disruption to workloads.
With commit 3aa565f53c39 ("powerpc/pseries: Add hooks to put the CPU
into an appropriate offline state") reverted, this code becomes
unnecessary, so remove it. Since any offline CPUs now are truly
offline from the platform's point of view, it is no longer necessary
to bring up CPUs only to have them call H_JOIN and then go offline
again upon resuming. Only active threads are required to call H_JOIN;
stopped threads can be left alone.

The ps3_mm_region_destroy() and ps3_mm_vas_destroy() routines
are called very late in the shutdown via kexec's mmu_cleanup_all
routine. By the time mmu_cleanup_all runs it is too late to use
udbg_printf, and calling it will cause PS3 systems to hang.
Remove all debugging statements from ps3_mm_region_destroy() and
ps3_mm_vas_destroy() and replace any error reporting with calls
to lv1_panic.
With this change builds with 'DEBUG' defined will not cause kexec
reboots to hang, and builds with 'DEBUG' defined or not will end
in lv1_panic if an error is encountered.

This was discovered developing qemu fwnmi sreset support. This
off-by-one bug means the last 16 bytes of the rtas area can not
be used for a 16 byte save area.
It's not a serious bug, and QEMU implementation has to retain a
workaround for old kernels, but it's good to tighten it.

Currently, we may perform a copy_to_user (through
simple_read_from_buffer()) while holding a context's register_lock,
while accessing the context save area.
This change uses a temporary buffer for the context save area data,
which we then pass to simple_read_from_buffer.

In the CPU-offline process, it calls mmdrop() after idle entry and the
subsequent call to cpuhp_report_idle_dead(). Once execution passes the
call to rcu_report_dead(), RCU is ignoring the CPU, which results in
lockdep complaining when mmdrop() uses RCU from either memcg or
debugobjects below.
Fix it by cleaning up the active_mm state from BP instead. Every arch
which has CONFIG_HOTPLUG_CPU should have already called idle_task_exit()
from AP. The only exception is parisc because it switches them to
&init_mm unconditionally (see smp_boot_one_cpu() and smp_cpu_init()),
but the patch will still work there because it calls mmgrab(&init_mm) in
smp_cpu_init() and then should call mmdrop(&init_mm) in finish_cpu().

export_imc_mode_and_cmd() function which creates the debugfs interface
for imc-mode and imc-command, is invoked when each nest pmu units is
registered.
When the first nest pmu unit is registered, export_imc_mode_and_cmd()
creates 'imc' directory under `/debug/powerpc/`. In the subsequent
invocations debugfs_create_dir() function returns, since the directory
already exists.
The recent commit <c33d442328f55> (debugfs: make error message a bit
more verbose), throws a warning if we try to invoke
`debugfs_create_dir()` with an already existing directory name.
Address this warning by making the debugfs directory registration in
the opal_imc_counters_probe() function, i.e invoke
export_imc_mode_and_cmd() function from the probe function.

Currently you can enable PPC_KUAP_DEBUG when PPC_KUAP is disabled,
even though the former has not effect without the latter.
Fix it so that PPC_KUAP_DEBUG can only be enabled when PPC_KUAP is

MCE handling on pSeries platform fails as recent rework to use common
code for pSeries and PowerNV in machine check error handling tries to
access per-cpu variables in realmode. The per-cpu variables may be
outside the RMO region on pSeries platform and needs translation to be
enabled for access. Just moving these per-cpu variable into RMO region
did'nt help because we queue some work to workqueues in real mode, which
again tries to touch per-cpu variables. Also fwnmi_release_errinfo()
cannot be called when translation is not enabled.
This patch fixes this by enabling translation in the exception handler
when all required real mode handling is done. This change only affects
the pSeries platform.
Without this fix below kernel crash is seen on injecting
SLB multihit:

machine_device_initcall expands to __define_machine_initcall, which in
turn has the macro machine_is used in it, which declares mach_##name

the warning.
To fix this, move define_machine after machine_device_initcall so that
the declaration occurs before the definition, which matches how
machine_device_initcall and define_machine work throughout
arch/powerpc.
While we're here, remove some spaces before tabs.

Taking closer look at the code, I can see that for_each_drmem_lmb is a
macro expanding into `for (lmb = &drmem_info->lmbs[0]; lmb <=
&drmem_info->lmbs[drmem_info->n_lmbs - 1]; lmb++)`. When drmem_info->lmbs
is NULL, the loop would iterate through the whole address range if it
weren't stopped by the NULL pointer dereference on the next line.
This patch aligns for_each_drmem_lmb and for_each_drmem_lmb_in_range
macro behavior with the common C semantics, where the end marker does
not belong to the scanned range, and alters get_lmb_range() semantics.
As a side effect, the wraparound observed in the crash is prevented.

An ioda_pe for each VF is allocated in pnv_pci_sriov_enable() before
the pci_dev for the VF is created. We need to set the pe->pdev pointer
at some point after the pci_dev is created. Currently we do that in:

pnv_pci_dma_dev_setup() (via phb->ops.dma_dev_setup)

The fixup needs to be done before setting up DMA for for the VF's PE,
but there's no real reason to delay it until this point. Move the
fixup into pnv_pci_ioda_fixup_iov() so the ordering is:
pcibios_add_device()
pnv_pci_ioda_fixup_iov() (via ppc_md.pcibios_fixup_sriov)
pcibios_bus_add_device()
...
This isn't strictly required, but it's a slightly more logical place
to do the fixup and it simplifies pnv_pci_dma_dev_setup().

On pseries there is a bug with adding hotplugged devices to an IOMMU
group. For a number of dumb reasons fixing that bug first requires
re-working how VFs are configured on PowerNV. For background, on
PowerNV we use the pcibios_sriov_enable() hook to do two things:
1. Create a pci_dn structure for each of the VFs, and
2. Configure the PHB's internal BARs so the MMIO range for each VF
maps to a unique PE.
Roughly speaking a PE is the hardware counterpart to a Linux IOMMU
group since all the devices in a PE share the same IOMMU table. A PE
also defines the set of devices that should be isolated in response to
a PCI error (i.e. bad DMA, UR/CA, AER events, etc). When isolated all
MMIO and DMA traffic to and from devicein the PE is blocked by the
root complex until the PE is recovered by the OS.
The requirement to block MMIO causes a giant headache because the P8
PHB generally uses a fixed mapping between MMIO addresses and PEs. As
a result we need to delay configuring the IOMMU groups for device
until after MMIO resources are assigned. For physical devices (i.e.
non-VFs) the PE assignment is done in pcibios_setup_bridge() which is
called immediately after the MMIO resources for downstream
devices (and the bridge's windows) are assigned. For VFs the setup is
more complicated because:
a) pcibios_setup_bridge() is not called again when VFs are activated, and
b) The pci_dev for VFs are created by generic code which runs after
pcibios_sriov_enable() is called.
The work around for this is a two step process:
1. A fixup in pcibios_add_device() is used to initialised the cached
pe_number in pci_dn, then
2. A bus notifier then adds the device to the IOMMU group for the PE
specified in pci_dn->pe_number.
A side effect fixing the pseries bug mentioned in the first paragraph
is moving the fixup out of pcibios_add_device() and into
pcibios_bus_add_device(), which is called much later. This results in
step 2. failing because pci_dn->pe_number won't be initialised when
the bus notifier is run.
We can fix this by removing the need for the fixup. The PE for a VF is
known before the VF is even scanned so we can initialise
pci_dn->pe_number pcibios_sriov_enable() instead. Unfortunately,
moving the initialisation causes two problems:
1. We trip the WARN_ON() in the current fixup code, and
2. The EEH core clears pdn->pe_number when recovering a VF and
relies on the fixup to correctly re-set it.
The only justification for either of these is a comment in
eeh_rmv_device() suggesting that pdn->pe_number *must* be set to
IODA_INVALID_PE in order for the VF to be scanned. However, this
comment appears to have no basis in reality. Both bugs can be fixed by
just deleting the code.

By default a pseries guest supports a H_PUT_TCE hypercall which maps
a single IOMMU page in a DMA window. Additionally the hypervisor may
support H_PUT_TCE_INDIRECT/H_STUFF_TCE which update multiple TCEs at once;
this is advertised via the device tree /rtas/ibm,hypertas-functions
property which Linux converts to FW_FEATURE_MULTITCE.
FW_FEATURE_MULTITCE is checked when dma_iommu_ops is used; however
the code managing the huge DMA window (DDW) ignores it and calls
H_PUT_TCE_INDIRECT even if it is explicitly disabled via
the "multitce=off" kernel command line parameter.
This adds FW_FEATURE_MULTITCE checking to the DDW code path.
This changes tce_build_pSeriesLP to take liobn and page size as
the huge window does not have iommu_table descriptor which usually
the place to store these numbers.

Commit e5afdf9dd515 ("powerpc/vfio_spapr_tce: Add reference counting to
iommu_table") missed an iommu_table allocation in the pseries vio code.
The iommu_table is allocated with kzalloc and as a result the associated
kref gets a value of zero. This has the side effect that during a DLPAR
remove of the associated virtual IOA the iommu_tce_table_put() triggers
a use-after-free underflow warning.

Further, since the refcount was always zero the iommu_tce_table_put()
fails to call the iommu_table release function resulting in a leak.
Fix this issue be initilizing the iommu_table kref immediately after
allocation.

String 'bus_desc.provider_name' allocated inside
papr_scm_nvdimm_init() will leaks in case call to
nvdimm_bus_register() fails or when papr_scm_remove() is called.
This minor patch ensures that 'bus_desc.provider_name' is freed in
error path for nvdimm_bus_register() as well as in papr_scm_remove().

This reverts commit edea902c1c1efb855f77e041f9daf1abe7a9768a.
At the time the change allowed direct DMA ops for secure VMs; however
since then we switched on using SWIOTLB backed with IOMMU (direct mapping)
and to make this work, we need dma_iommu_ops which handles all cases
including TCE mapping I/O pages in the presence of an IOMMU.

On PowerNV the PCIe topology is (currently) managed by the powernv platform
code in Linux in cooperation with the platform firmware. Linux's native
PCIe port service drivers operate independently of both and this can cause
problems.
The main issue is that the portbus driver will conflict with the platform
specific hotplug driver (pnv_php) over ownership of the MSI used to notify
the host when a hotplug event occurs. The portbus driver claims this MSI on
behalf of the individual port services because the same interrupt is used
for hotplug events, PMEs (on root ports), and link bandwidth change
notifications. The portbus driver will always claim the interrupt even if
the individual port service drivers, such as pciehp, are compiled out.
The second, bigger, problem is that the hotplug port service driver
fundamentally does not work on PowerNV. The platform assumes that all
PCI devices have a corresponding arch-specific handle derived from the DT
node for the device (pci_dn) and without one the platform will not allow
a PCI device to be enabled. This problem is largely due to historical
baggage, but it can't be resolved without significant re-factoring of the
platform PCI support.
We can fix these problems in the interim by setting the
"pcie_ports_disabled" flag during platform initialisation. The flag
indicates the platform owns the PCIe ports which stops the portbus driver
from being registered.
This does have the side effect of disabling all port services drivers
that is: AER, PME, BW notifications, hotplug, and DPC. However, this is
not a huge disadvantage on PowerNV since these services are either unused
or handled through other means.

With commit 247f2f6f3c70 ("sched/core: Don't schedule threads on
pre-empted vCPUs"), the scheduler avoids preempted vCPUs to schedule
tasks on wakeup. This leads to wrong choice of CPU, which in-turn
leads to larger wakeup latencies. Eventually, it leads to performance
regression in latency sensitive benchmarks like soltp, schbench etc.
On Powerpc, vcpu_is_preempted() only looks at yield_count. If the
yield_count is odd, the vCPU is assumed to be preempted. However
yield_count is increased whenever the LPAR enters CEDE state (idle).
So any CPU that has entered CEDE state is assumed to be preempted.
Even if vCPU of dedicated LPAR is preempted/donated, it should have
right of first-use since they are supposed to own the vCPU.
