update_pasid() and its call chain are currently unused in the tree because
Thomas disabled the ENQCMD feature. The feature will be re-enabled shortly
using a different approach and update_pasid() and its call chain will not
be used in the new approach.
Remove the useless functions.

The microcode loader has been looping through __start_builtin_fw down to
__end_builtin_fw to look for possibly built-in firmware for microcode
updates.
Now that the firmware loader code has exported an API for looping
through the kernel's built-in firmware section, use it and drop the x86
implementation in favor.

Should instruction emulation fail, include the VM exit reason, etc. in
the emulation_failure data passed to userspace, in order that the VMM
can report it as a debugging aid when describing the failure.

Extend the get_exit_info static call to provide the reason for the VM
exit. Modify relevant trace points to use this rather than extracting
the reason in the caller.

Use a rw_semaphore instead of a mutex to coordinate APICv updates so that
vCPUs responding to requests can take the lock for read and run in
parallel. Using a mutex forces serialization of vCPUs even though
kvm_vcpu_update_apicv() only touches data local to that vCPU or is
protected by a different lock, e.g. SVM's ir_list_lock.

Paul pointed out the error messages when KVM fails to load are unhelpful
in understanding exactly what went wrong if userspace probes the "wrong"
module.
Add a mandatory kvm_x86_ops field to track vendor module names, kvm_intel
and kvm_amd, and use the name for relevant error message when KVM fails
to load so that the user knows which module failed to load.
Opportunistically tweak the "disabled by bios" error message to clarify
that _support_ was disabled, not that the module itself was magically
disabled by BIOS.

SDM section 18.2.3 mentioned that:
"IA32_PERF_GLOBAL_OVF_CTL MSR allows software to clear overflow indicator(s) of
any general-purpose or fixed-function counters via a single WRMSR."
It is R/W mentioned by SDM, we read this msr on bare-metal during perf testing,
the value is always 0 for ICX/SKX boxes on hands. Let's fill get_msr
MSR_CORE_PERF_GLOBAL_OVF_CTRL w/ 0 as hardware behavior and drop
global_ovf_ctrl variable.

Unify the flags for rmaps and page tracking data, using a
single flag in struct kvm_arch and a single loop to go
over all the address spaces and memslots. This avoids
code duplication between alloc_all_memslots_rmaps and
kvm_page_track_enable_mmu_write_tracking.

To date, VMM-directed TSC synchronization and migration has been a bit
messy. KVM has some baked-in heuristics around TSC writes to infer if
the VMM is attempting to synchronize. This is problematic, as it depends
on host userspace writing to the guest's TSC within 1 second of the last
write.
A much cleaner approach to configuring the guest's views of the TSC is to
simply migrate the TSC offset for every vCPU. Offsets are idempotent,
and thus not subject to change depending on when the VMM actually
reads/writes values from/to KVM. The VMM can then read the TSC once with
KVM_GET_CLOCK to capture a (realtime, host_tsc) pair at the instant when
the guest is paused.

Protect the reference point for kvmclock with a seqcount, so that
kvmclock updates for all vCPUs can proceed in parallel. Xen runstate
updates will also run in parallel and not bounce the kvmclock cacheline.
Of the variables that were protected by pvclock_gtod_sync_lock,
nr_vcpus_matched_tsc is different because it is updated outside
pvclock_update_vm_gtod_copy and read inside it. Therefore, we
need to keep it protected by a spinlock. In fact it must now
be a raw spinlock, because pvclock_update_vm_gtod_copy, being the
write-side of a seqcount, is non-preemptible. Since we already
have tsc_write_lock which is a raw spinlock, we can just use
tsc_write_lock as the lock that protects the write-side of the
seqcount.

Handling the migration of TSCs correctly is difficult, in part because
Linux does not provide userspace with the ability to retrieve a (TSC,
realtime) clock pair for a single instant in time. In lieu of a more
convenient facility, KVM can report similar information in the kvm_clock
structure.
Provide userspace with a host TSC & realtime pair iff the realtime clock
is based on the TSC. If userspace provides KVM_SET_CLOCK with a valid
realtime value, advance the KVM clock by the amount of elapsed time. Do
not step the KVM clock backwards, though, as it is a monotonic
oscillator.

Avoid allocating the gfn_track arrays if nothing needs them. If there
are no external to KVM users of the API (i.e. no GVT-g), then page
tracking is only needed for shadow page tables. This means that when tdp
is enabled and there are no external users, then the gfn_track arrays
can be lazily allocated when the shadow MMU is actually used. This avoid
allocations equal to .05% of guest memory when nested virtualization is
not used, if the kernel is compiled without GVT-g.

By switching from kfree() to kvfree() in kvm_arch_free_vm() Arm64 can
use the common variant. This can be accomplished by adding another
macro __KVM_HAVE_ARCH_VM_FREE, which will be used only by x86 for now.
Further simplification can be achieved by adding __kvm_arch_free_vm()
doing the common part.

mmu_try_to_unsync_pages checks if page tracking is active for the given
gfn, which requires knowing the memslot. We can pass down the memslot
via make_spte to avoid this lookup.
The memslot is also handy for make_spte's marking of the gfn as dirty:
we can test whether dirty page tracking is enabled, and if so ensure that
pages are mapped as writable with 4K granularity. Apart from the warning,
no functional change is intended.

Updates to the kvmclock parameters needs to do a complicated dance of
KVM_REQ_MCLOCK_INPROGRESS and KVM_REQ_CLOCK_UPDATE in addition to taking
pvclock_gtod_sync_lock. Place that in two functions that can be called
on all of master clock update, KVM_SET_CLOCK, and Hyper-V reenlightenment.

Open code the call to mmu->translate_gpa() when loading nested PDPTRs and
kill off the existing helper, kvm_read_guest_page_mmu(), to discourage
incorrect use. Reading guest memory straight from an L2 GPA is extremely
rare (as evidenced by the lack of users), as very few constructs in x86
specify physical addresses, even fewer are virtualized by KVM, and even
fewer yet require emulation of L2 by L0 KVM.
No functional change intended.

number of allowed vcpu-ids. This has already led to confusion, so
rename KVM_MAX_VCPU_ID to KVM_MAX_VCPU_IDS to make its semantics more

kvm_make_vcpus_request_mask() already disables preemption so just like
kvm_make_all_cpus_request_except() it can be switched to using
pre-allocated per-cpu cpumasks. This allows for improvements for both
users of the function: in Hyper-V emulation code 'tlb_flush' can now be
dropped from 'struct kvm_vcpu_hv' and kvm_make_scan_ioapic_request_mask()
gets rid of dynamic allocation.
cpumask_available() checks in kvm_make_vcpu_request() and
kvm_kick_many_cpus() can now be dropped as they checks for an impossible
condition: kvm_init() makes sure per-cpu masks are allocated.

Hyperv provides GHCB protocol to write Synthetic Interrupt
Controller MSR registers in Isolation VM with AMD SEV SNP
and these registers are emulated by hypervisor directly.
Hyperv requires to write SINTx MSR registers twice. First
writes MSR via GHCB page to communicate with hypervisor
and then writes wrmsr instruction to talk with paravisor
which runs in VMPL0. Guest OS ID MSR also needs to be set
via GHCB page.

memory visible to host. Call it inside set_memory_decrypted

Pull x86 core updates from Borislav Petkov:
- Do not #GP on userspace use of CLI/STI but pretend it was a NOP to

The size of the exception stacks was increased by the commit in Fixes,
resulting in stack sizes greater than a page in size. The #VC exception
handling was only mapping the first (bottom) page, resulting in an
SEV-ES guest failing to boot.
Make the #VC exception stacks part of the default exception stacks
storage and allocate them with a CONFIG_AMD_MEM_ENCRYPT=y .config. Map
them only when a SEV-ES guest has been detected.
Rip out the custom VC stacks mapping and storage code.

It turns out that a single page of stack is trivial to overflow with
all the tracing gunk enabled. Raise the exception stacks to 2 pages,
which is still half the interrupt stacks, which are at 4 pages.

Current code has an explicit check for hitting the task stack guard;
but overflowing any of the other stacks will get you a non-descript
general #DF warning.
Improve matters by using get_stack_info_noinstr() to detetrmine if and
which stack guard page got hit, enabling a better stack warning.
In specific, Michael Wang reported what turned out to be an NMI
exception stack overflow, which is now clearly reported as such:

Since commit c8137ace5638 ("x86/iopl: Restrict iopl() permission
scope") it's possible to emulate iopl(3) using ioperm(), except for
the CLI/STI usage.
Userspace CLI/STI usage is very dubious (read broken), since any
exception taken during that window can lead to rescheduling anyway (or
worse). The IOPL(2) manpage even states that usage of CLI/STI is highly
discouraged and might even crash the system.
Of course, that won't stop people and HP has the dubious honour of
being the first vendor to be found using this in their hp-health
package.
In order to enable this 'software' to still 'work', have the #GP treat
the CLI/STI instructions as NOPs when iopl(3). Warn the user that
their program is doing dubious things.

Pull tracing updates from Steven Rostedt:
- kprobes: Restructured stack unwinder to show properly on x86 when a
stack dump happens from a kretprobe callback.
- Fix to bootconfig parsing
- Have tracefs allow owner and group permissions by default (only
denying others). There's been pressure to allow non root to tracefs
in a controlled fashion, and using groups is probably the safest.
- Bootconfig memory managament updates.
- Bootconfig clean up to have the tools directory be less dependent on
changes in the kernel tree.
- Allow perf to be traced by function tracer.
- Rewrite of function graph tracer to be a callback from the function
tracer instead of having its own trampoline (this change will happen
on an arch by arch basis, and currently only x86_64 implements it).
- Allow multiple direct trampolines (bpf hooks to functions) be batched
together in one synchronization.
- Allow histogram triggers to add variables that can perform
calculations against the event's fields.
- Use the linker to determine architecture callbacks from the ftrace
trampoline to allow for proper parameter prototypes and prevent
warnings from the compiler.
- Extend histogram triggers to key off of variables.
- Have trace recursion use bit magic to determine preempt context over
if branches.
- Have trace recursion disable preemption as all use cases do anyway.
- Added testing for verification of tracing utilities.
- Various small clean ups and fixes.

We don't need special hook for graph tracer entry point,
but instead we can use graph_ops::func function to install
the return_hooker.
This moves the graph tracing setup _before_ the direct
trampoline prepares the stack, so the return_hooker will
be called when the direct trampoline is finished.
This simplifies the code, because we don't need to take into
account the direct trampoline setup when preparing the graph
tracer hooker and we can allow function graph tracer on entries
registered with direct trampoline.

Since the kretprobe replaces the function return address with
the kretprobe_trampoline on the stack, x86 unwinders can not
continue the stack unwinding at that point, or record
kretprobe_trampoline instead of correct return address.
To fix this issue, find the correct return address from task's
kretprobe_instances as like as function-graph tracer does.
With this fix, the unwinder can correctly unwind the stack
from kretprobe event on x86, as below.

Add UNWIND_HINT_FUNC on __kretprobe_trampoline() code so that ORC

The __kretprobe_trampoline_handler() callback, called from low level
arch kprobes methods, has the 'trampoline_address' parameter, which is
entirely superfluous as it basically just replicates:
dereference_kernel_function_descriptor(kretprobe_trampoline)
In fact we had bugs in arch code where it wasn't replicated correctly.
So remove this superfluous parameter and use kretprobe_trampoline_addr()
instead.

Hyper-V needs to issue the GHCB HV call in order to read/write MSRs in
Isolation VMs. For that, expose sev_es_ghcb_hv_call().
The Hyper-V Isolation VMs are unenlightened guests and run a paravisor
at VMPL0 for communicating. GHCB pages are being allocated and set up
by that paravisor. Linux gets the GHCB page's physical address via
MSR_AMD64_SEV_ES_GHCB from the paravisor and should not change it.
Add a @set_ghcb_msr parameter to sev_es_ghcb_hv_call() to control
whether the function should set the GHCB's address prior to the call or
not and export that function for use by HyperV.

Pull x86 cpu updates from Borislav Petkov:
- Start checking a CPUID bit on AMD Zen3 which states that the CPU
clears the segment base when a null selector is written. Do the
explicit detection on older CPUs, zen2 and hygon specifically, which
have the functionality but do not advertize the CPUID bit. Factor in
the presence of a hypervisor underneath the kernel and avoid doing
the explicit check there which the HV might've decided to not
advertize for migration safety reasons, or similar.
- Add support for a new X86 CPU vendor: VORTEX. Needed for whitelisting
those CPUs in the hardware vulnerabilities detection
- Force the compiler to use rIP-relative addressing in the fallback
path of static_cpu_has(), in order to avoid unnecessary register
pressure
