After commit "TTY: move tty buffers to tty_port", the tty buffers are
not freed in some drivers. This is because tty_port_destructor is not
called whenever a tty_port is freed. This was an assumption I counted
with but was unfortunately untrue. So fix the drivers to fulfil this
assumption.
To be sure, the TTY buffers (and later some stuff) are gone along with
the tty_port, we have to call tty_port_destroy at tear-down places.
This is mostly where the structure containing a tty_port is freed.
This patch does exactly that -- put tty_port_destroy at those places.

Pull TTY changes from Greg Kroah-Hartman:
"As we skipped the merge window for 3.6-rc1 for the tty tree,
everything is now settled down and working properly, so we are ready
for 3.7-rc1. Here's the patchset, it's big, but the large changes are
removing a firmware file and adding a staging tty driver (it depended
on the tty core changes, so it's going through this tree instead of
the staging tree.)
All of these patches have been in the linux-next tree for a while.

In most of the time, the driver needs to check if the cts flow control
is enabled. But now, the driver checks the ASYNC_CTS_FLOW flag manually,
which is not a grace way. So add a new wraper function to make the code
tidy and clean.

Commit "TTY: synclink_cs, use dynamic tty devices" added a call to
tty_port_register_device with a proper device as the last argument.
But it was not correct and it causes build failures:
synclink_cs.c: In function ‘mgslpc_add_device’:
synclink_cs.c:2735:16: error: request for member ‘dev’ in something not a structure or union
info->p_dev is a pointer, so act as that.
I wonder why my build scripts did not notice. I have to re-check them.

This allows us to provide the tty layer with information about
tty_port for each link. And it also allows us to get rid of the
remove_device loop in synclink_cs_exit because we had to reorder
pcmcia and tty driver registration in init. This was because we need
to have serial_driver initialized when calling
tty_port_register_device from pcmcia ->probe.

We will need to change the order of tty and pcmcia drivers
initializations (see the reason later in this series). And the fail
path handling is currently performed in a separate function that as
well takes care of proper deinitialization in module_exit. It is hard
to read and will need to be adjusted by our changes anyway. Instead,
get rid of this helper function and do the fail paths handling
directly in the init function. (And move the body of the function to
module_exit.)

tty_port_tty_get() can return NULL after port hangup that may happen anytime.
The patch adds checks that tty_port_tty_get() returns nonNULL around places
where tty is actually used.
I have no actual hardware to test the patch, so I have updated rx side
processing from common sense only.
v2: rx handling updated according Alan Cox feedback.
Found by Linux Driver Verification project (linuxtesting.org).

Checking if tty->index is in bounds is not needed. The tty has the
index set in the initial open. This is done in get_tty_driver. And it
can be only in interval <0,driver->num).
So remove the tests which check exactly this interval. Some are
left untouched as they check against the current backing device count.
(Leaving apart that the check is racy in most of the cases.)

All num, magic and owner are set by alloc_tty_driver. No need to
re-set them on each allocation site.
pti driver sets something different to what it passes to
alloc_tty_driver. It is not a bug, since we don't use the lines
parameter in any way. Anyway this is fixed, and now we do the right
thing.

module_param(bool) used to counter-intuitively take an int. In
fddd5201 (mid-2009) we allowed bool or int/unsigned int using a messy
trick.
It's time to remove the int/unsigned int option. For this version
it'll simply give a warning, but it'll break next kernel version.

We don't actually need this and it causes problems for internal use of
this functionality. Currently there is a single use of the FILE * pointer.
That is the serial core which uses it to check tty_hung_up_p. However if
that is true then IO_ERROR is also already set so the check may be removed.

release_region/release_mem_region
Request_region should be used with release_region, not release_resource.
This patch contains a number of changes, related to calls to request_region,
request_mem_region, and the associated error handling code.
1. For the call to request_region, the variable io_resource storing the
result is dropped. The call to release_resource at the end of the function
is changed to a call to release_region with the first two arguments of
request_region as its arguments. The same call to release_region is also
added to release_ipwireless.
2. The first call to request_mem_region is now tested and ret is set to
-EBUSY if the the call has failed. This call was associated with the
initialization of ipw->attr_memory. But the error handling code was
testing ipw->common_memory. The definition of release_ipwireless also
suggests that this call should be associated with ipw->common_memory, not
ipw->attr_memory.
3. The second call to request_mem_region is now tested and ret is
set to -EBUSY if the the call has failed.
4. The various gotos to the error handling code is adjusted so that there
is no need for ifs.
5. Return the value stored in the ret variable rather than -1.

All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.
The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.
New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time. Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.
The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.
Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

All these files use the big kernel lock in a trivial
way to serialize their private file operations,
typically resulting from an earlier semi-automatic
pushdown from VFS.
None of these drivers appears to want to lock against
other code, and they all use the BKL as the top-level
lock in their file operations, meaning that there
is no lock-order inversion problem.
Consequently, we can remove the BKL completely,
replacing it with a per-file mutex in every case.
Using a scripted approach means we can avoid
typos.
These drivers do not seem to be under active
maintainance from my brief investigation. Apologies
to those maintainers that I have missed.

When CONF_AUTO_SET_IO or CONF_AUTO_SET_IOMEM are set, the corresponding
fields in struct pcmcia_device *p_dev->resource[0,1,2] are set
accordinly. Drivers wishing to override certain settings may do so in
the callback function, but they no longer need to parse the CIS entries

pcmcia_enable_device() now replaces pcmcia_request_configuration().
Instead of config_req_t, all necessary flags are either passed as
a parameter to pcmcia_enable_device(), or (in rare circumstances)
set in struct pcmcia_device -> flags.
With the last remaining user of include/pcmcia/cs.h gone, remove
all references.

IntType was only set to INT_MEMORY (driver pcmciamtd) or INT_MEMORY_AND_IO
(all other drivers). As this flags seems to relate to ioport access, make
it conditional to the driver having requested IO port access. There are two
drivers which do not request IO ports, but did set INT_MEMORY_AND_IO:
ray_cs and b43. For those, we consistently only set INT_MEMORY in future.

Instead of win_req_t, drivers are now requested to fill out
struct pcmcia_device *p_dev->resource[2,3,4,5] for up to four iomem
ranges. After a call to pcmcia_request_window(), the windows found there
are reserved and may be used until pcmcia_release_window() is called.
