because the allocation could fail from iommu_map_page(), and the volume
of this call could be huge which may generate a lot of serial console
output and cosumes all CPUs.
Fix it by silencing the warning in this call site, and there is still a
dev_err() later to notify the failure.

After the conversion to lock-less dma-api call the
increase_address_space() function can be called without any
locking. Multiple CPUs could potentially race for increasing
the address space, leading to invalid domain->mode settings
and invalid page-tables. This has been happening in the wild
under high IO load and memory pressure.
Fix the race by locking this operation. The function is
called infrequently so that this does not introduce
a performance regression in the dma-api path again.

When devices are attached to the amd_iommu in a kdump kernel, the old device
table entries (DTEs), which were copied from the crashed kernel, will be
overwritten with a new domain number. When the new DTE is written, the IOMMU
is told to flush the DTE from its internal cache--but it is not told to flush
the translation cache entries for the old domain number.
Without this patch, AMD systems using the tg3 network driver fail when kdump
tries to save the vmcore to a network system, showing network timeouts and
(sometimes) IOMMU errors in the kernel log.
This patch will flush IOMMU translation cache entries for the old domain when
a DTE gets overwritten with a new domain number.

If a device has an exclusion range specified in the IVRS
table, this region needs to be reserved in the iova-domain
of that device. This hasn't happened until now and can cause
data corruption on data transfered with these devices.
Treat exclusion ranges as reserved regions in the iommu-core
to fix the problem.

Take into account that sg->offset can be bigger than PAGE_SIZE when

When a VM is terminated, the VFIO driver detaches all pass-through
devices from VFIO domain by clearing domain id and page table root
pointer from each device table entry (DTE), and then invalidates
the DTE. Then, the VFIO driver unmap pages and invalidate IOMMU pages.
Currently, the IOMMU driver keeps track of which IOMMU and how many
devices are attached to the domain. When invalidate IOMMU pages,
the driver checks if the IOMMU is still attached to the domain before
issuing the invalidate page command.
However, since VFIO has already detached all devices from the domain,
the subsequent INVALIDATE_IOMMU_PAGES commands are being skipped as
there is no IOMMU attached to the domain. This results in data
corruption and could cause the PCI device to end up in indeterministic
state.
Fix this by invalidate IOMMU pages when detach a device, and
before decrementing the per-domain device reference counts.

In the error path of map_sg there is an incorrect if condition
for breaking out of the loop that searches the scatterlist
for mapped pages to unmap. Instead of breaking out of the
loop once all the pages that were mapped have been unmapped,
it will break out of the loop after it has unmapped 1 page.
Fix the condition, so it breaks out of the loop only after
all the mapped pages have been unmapped.

address instead of the pfn. This results in a bad value getting into

The parameter is still there but it's ignored. We need to check its
value before deciding to go into passthrough mode for AMD IOMMU v2
capable device.
We occasionally use this parameter to force v2 capable device into
translation mode to debug memory corruption that we suspect is
caused by DMA writes.
To address the following comment from Joerg Roedel on the first
version, v2 capability of device is completely ignored.
> This breaks the iommu_v2 use-case, as it needs a direct mapping for the
> devices that support it.
And from Documentation/admin-guide/kernel-parameters.txt:
This option does not override iommu=pt

ACPI HID devices do not actually have an alias for
them in the IVRS. But dev_data->alias is still used
for indexing into the IOMMU device table for devices
being handled by the IOMMU. So for ACPI HID devices,
we simply return the corresponding devid as an alias,
as parsed from IVRS table.

Boris Ostrovsky reported a memory leak with device passthrough when SME
is active.
The VFIO driver uses iommu_iova_to_phys() to get the physical address for
an iova. This physical address is later passed into vfio_unmap_unpin() to
unpin the memory. The vfio_unmap_unpin() uses pfn_valid() before unpinning
the memory. The pfn_valid() check was failing because encryption mask was
part of the physical address returned. This resulted in the memory not
being unpinned and therefore leaked after the guest terminates.
The memory encryption mask must be cleared from the physical address in
iommu_iova_to_phys().

Although the mapping has already been removed in the page table, it maybe
still exist in TLB. Suppose the freed IOVAs is reused by others before the

find_dev_data() does not check whether the return value alloc_dev_data()
is NULL. This was okay once because the pointer was returned once as-is.
Since commit df3f7a6e8e85 ("iommu/amd: Use is_attach_deferred
call-back") the pointer may be used within find_dev_data() so a NULL
check is required.

When SME memory encryption is active it will rely on SWIOTLB to handle
DMA for devices that cannot support the addressing requirements of
having the encryption mask set in the physical address. The IOMMU
currently disables SWIOTLB if it is not running in passthrough mode.
This is not desired as non-PCI devices attempting DMA may fail. Update
the code to check if SME is active and not disable SWIOTLB.

In get_domain(), 'domain' could be NULL before it's passed to dma_ops_domain()
to dereference. And the current code calling get_domain() can't deal with the
returned 'domain' well if its value is NULL.
So before dma_ops_domain() calling, check if 'domain' is NULL, If yes just return
ERR_PTR(-EBUSY) directly.

When iommu is pre_enabled in kdump kernel, if a device is set up with
guest translations (DTE.GV=1), then don't copy GCR3 table root pointer
but move the device over to an empty guest-cr3 table and handle the
faults in the PPR log (which answer them with INVALID). After all these
PPR faults are recoverable for the device and we should not allow the
device to change old-kernels data when we don't have to.

Add function copy_dev_tables to copy the old DEV table entries of the panicked
kernel to the new allocated device table. Since all iommus share the same device
table the copy only need be done one time. Here add a new global old_dev_tbl_cpy
to point to the newly allocated device table which the content of old device
table will be copied to. Besides, we also need to:
- Check whether all IOMMUs actually use the same device table with the same size
- Verify that the size of the old device table is the expected size.
- Reserve the old domain id occupied in 1st kernel to avoid touching the old
io-page tables. Then on-flight DMA can continue looking it up.
And also define MACRO DEV_DOMID_MASK to replace magic number 0xffffULL, it can be
reused in copy_dev_tables().

In AMD-Vi spec several bits of IO PTE fields and DTE fields are similar
so that both of them can share the same MACRO definition. However
defining them respectively can make code more read-able. Do it now.

IRTE[GALogIntr] bit should set when enabling guest_mode, which enables
IOMMU to generate entry in GALog when IRTE[IsRun] is not set, and send
an interrupt to notify IOMMU driver.

The IOMMU is programmed with physical addresses for the various tables
and buffers that are used to communicate between the device and the
driver. When the driver allocates this memory it is encrypted. In order
for the IOMMU to access the memory as encrypted the encryption mask needs
to be included in these physical addresses during configuration.
The PTE entries created by the IOMMU should also include the encryption
mask so that when the device behind the IOMMU performs a DMA, the DMA
will be performed to encrypted memory.

The code suffered from very high flush rates, with the new
implementation the flush rate is down to ~1% of what it was before

The queue flushing is pretty inefficient when it flushes the
queues for all cpus at once. Further it flushes all domains
from all IOMMUs for all CPUs, which is overkill as well.
Rip it out to make room for something more efficient.

Currently if there is no room to add a command to the command buffer, the
driver performs a "completion wait" which only returns when all commands
on the queue have been processed. There is no need to wait for the entire
command queue to be executed before adding the next command.
Update the driver to perform the same udelay() loop that the "completion
wait" performs, but instead re-read the head pointer to determine if
sufficient space is available. The very first time it is found that there
is no space available, the udelay() will be skipped to immediately perform
the opportunistic read of the head pointer. If it is still found that
there is not sufficient space, then the udelay() will be performed.

As newer, higher speed devices are developed, perf data shows that the
amount of MMIO that is performed when submitting commands to the IOMMU
causes performance issues. Currently, the command submission path reads
the command buffer head and tail pointers and then writes the tail
pointer once the command is ready.
The tail pointer is only ever updated by the driver so it can be tracked
by the driver without having to read it from the hardware.
The head pointer is updated by the hardware, but can be read
opportunistically. Reading the head pointer only when it appears that
there might not be room in the command buffer and then re-checking the
available space reduces the number of times the head pointer has to be
read.

Misbehaving devices can cause an endless chain of
io-page-faults, flooding dmesg and making the system-log
unusable or even prevent the system from booting.
So ratelimit the error messages about io-page-faults on a
per-device basis.
