If m5602_write_bridge times out, it will return a negative error value.
So properly check for this and handle the error correctly instead of
just ignoring it.

This reverts commit 656025850074f5c1ba2e05be37bda57ba2b8d491.
Because of recent interactions with developers from @umn.edu, all
commits from them have been recently re-reviewed to ensure if they were
correct or not.
Upon review, this commit was found to be incorrect for the reasons
below, so it must be reverted. It will be fixed up "correctly" in a
later kernel change.
Different error values should never be "OR" together and expect anything
sane to come out of the result.

In mt9m111_probe, m5602_write_bridge can timeout and return a negative
error value. The fix checks for this error and passes it upstream.

Use a more typical logging style.
The current macro hides the gspca_dev argument so add it to the
macro uses instead.
Miscellanea:
o Add missing '\n' terminations to formats
o Realign arguments to open parenthesis
o Remove commented out uses of PDEBUG

The header file has some private static structures that
are used only by the C file. Move those structures to the C file,
in order to shut up gcc 6.1 warnings.

Store complete current mode (struct v4l2_pix_format) in struct gspca_dev
instead of separate pixfmt, width and height parameters.
This is a preparation for variable resolution support.

Instead use v4l2_dbg and v4l2_err. Note that the PDEBUG macro is kept to
make this patch-set less invasive, but it is simply a wrapper around
v4l2_dbg now. Most of the other changes are there to make the dev parameter
for the v4l2_xxx macros available everywhere we do logging.
