Implement NVMe-oF In-Band authentication according to NVMe TPAR 8006.
This patch adds two new fabric options 'dhchap_secret' to specify the
pre-shared key (in ASCII respresentation according to NVMe 2.0 section
8.13.5.8 'Secret representation') and 'dhchap_ctrl_secret' to specify
the pre-shared controller key for bi-directional authentication of both
the host and the controller.
Re-authentication can be triggered by writing the PSK into the new
controller sysfs attribute 'dhchap_secret' or 'dhchap_ctrl_secret'.

Apple SoCs such as the M1 come with an embedded NVMe controller that
is not attached to any PCIe bus. Additionally, it doesn't conform
to the NVMe specification and requires a bunch of changes to command
submission and IOMMU configuration to work.

selecting both, have the selection transitive such that NVME_FABRICS

When NVME_TCP is enabled and CRYPTO is disabled, it results in the
following Kbuild warning:
WARNING: unmet direct dependencies detected for CRYPTO_CRC32C
Depends on [n]: CRYPTO [=n]
Selected by [y]:
- NVME_TCP [=y] && INET [=y] && BLK_DEV_NVME [=y]
The reason is that NVME_TCP selects CRYPTO_CRC32C without depending on or
selecting CRYPTO while CRYPTO_CRC32C is subordinate to CRYPTO.
Honor the kconfig menu hierarchy to remove kconfig dependency warnings.

Since commit 84af7a6194e4 ("checkpatch: kconfig: prefer 'help' over
'---help---'"), the number of '---help---' has been gradually
decreasing, but there are still more than 2400 instances.
This commit finishes the conversion. While I touched the lines,
I also fixed the indentation.
There are a variety of indentation styles found.
a) 4 spaces + '---help---'
b) 7 spaces + '---help---'
c) 8 spaces + '---help---'
d) 1 space + 1 tab + '---help---'
e) 1 tab + '---help---' (correct indentation)
f) 1 tab + 1 space + '---help---'
g) 1 tab + 2 spaces + '---help---'
In order to convert all of them to 1 tab + 'help', I ran the
following commend:

I see no good reason for the "If unsure, say N" advice in the description
of the NVME_HWMON configuration option. It is not dangerous, it does
not select any other option, and has a fairly low overhead.
As the option is already not enabled by default, further suggesting
hesitant users to not enable it is not useful anyway. Unlike some other
options where the description alone may not be sufficient for users to
make a decision, NVME_HWMON is pretty simple to grasp in my opinion,
so just let the user do what they want.

Currently t10-pi can only be built into the block layer which via
crc-t10dif pulls in a whole chunk of the Crypto API. In fact all
users of t10-pi work as modules and there is no reason for it to
always be built-in.
This patch adds a new hidden option for t10-pi that is selected
automatically based on BLK_DEV_INTEGRITY and whether the users
of t10-pi are built-in or not.

nvme devices report temperature information in the controller information
(for limits) and in the smart log. Currently, the only means to retrieve
this information is the nvme command line interface, which requires
super-user privileges.
At the same time, it would be desirable to be able to use NVMe temperature
information for thermal control.
This patch adds support to read NVMe temperatures from the kernel using the
hwmon API and adds temperature zones for NVMe drives. The thermal subsystem
can use this information to set thermal policies, and userspace can access
it using libsensors and/or the "sensors" command.
Example output from the "sensors" command:

This patch implements the NVMe over TCP host driver. It can be used to
connect to remote NVMe over Fabrics subsystems over good old TCP/IP.
The driver implements the TP 8000 of how nvme over fabrics capsules and
data are encapsulated in nvme-tcp pdus and exchaged on top of a TCP byte
stream. nvme-tcp header and data digest are supported as well.
To connect to all NVMe over Fabrics controllers reachable on a given taget
port over TCP use the following command:
nvme connect-all -t tcp -a $IPADDR
This requires the latest version of nvme-cli with TCP support.

Several subsystems depend on INFINIBAND_ADDR_TRANS, which in turn depends
on INFINIBAND. However, when with CONFIG_INIFIBAND=m, this leads to a
link error when another driver using it is built-in. The
INFINIBAND_ADDR_TRANS dependency is insufficient here as this is
a 'bool' symbol that does not force anything to be a module in turn.

INFINIBAND_ADDR_TRANS depends on INFINIBAND. So there's no need for
options which depend INFINIBAND_ADDR_TRANS to also depend on INFINIBAND.

This patch adds native multipath support to the nvme driver. For each
namespace we create only single block device node, which can be used
to access that namespace through any of the controllers that refer to it.
The gendisk for each controllers path to the name space still exists
inside the kernel, but is hidden from userspace. The character device
nodes are still available on a per-controller basis. A new link from
the sysfs directory for the subsystem allows to find all controllers
for a given subsystem.
Currently we will always send I/O to the first available path, this will
be changed once the NVMe Asynchronous Namespace Access (ANA) TP is
ratified and implemented, at which point we will look at the ANA state
for each namespace. Another possibility that was prototyped is to
use the path that is closes to the submitting NUMA code, which will be
mostly interesting for PCI, but might also be useful for RDMA or FC
transports in the future. There is not plan to implement round robin
or I/O service time path selectors, as those are not scalable with
the performance rates provided by NVMe.
The multipath device will go away once all paths to it disappear,
any delay to keep it alive needs to be implemented at the controller
level.

The SCSI-to-NVMe translations were added to assist storage applications
utilizing SG_IO transitioning to NVMe. It was always recommended,
however, to use native NVMe for device management as too much is lost
in translation and the maintenance burden in keeping this kludgey
layer around has been neglected such that much of the translations are
completely broken.
This patch removes SG_IO handling from NVMe to avoid any confusion
regarding maintenance support for this interface. The config option for
NVMe SCSI emulation has been disabled by default since 4.5. The driver
has supported native nvme user commands since the beginning, and native
tooling is publicly available for use or as reference for anyone writing
their own tools, so there's no excuse for hanging onto a broken crutch.

Implements the FC-NVME T11 definition of how nvme fabric capsules are
performed on an FC fabric. Utilizes a lower-layer API to FC host adapters
to send/receive FC-4 LS operations and FCP operations that comprise NVME
over FC operation.
The T11 definitions for FC-4 Link Services are implemented which create
NVMeOF connections. Implements the hooks with blk-mq to then submit admin
and io requests to the different connections.

Commit aa71987472a9 ("nvme: fabrics drivers don't need the nvme-pci
driver") removed the dependency on BLK_DEV_NVME, but the cdoe does
depend on the block layer (which used to be an implicit dependency
through BLK_DEV_NVME).
Otherwise you get various errors from the kbuild test robot random
config testing when that happens to hit a configuration with BLOCK
device support disabled.

This patch implements the RDMA host (initiator in SCSI speak) driver. It
can be used to connect to remote NVMe over Fabrics controllers over
Infiniband, RoCE or iWarp, and uses the existing NVMe core driver as well
a the new fabrics library.
To connect to all NVMe over Fabrics controller reachable on a given taget
port using RDMA/CM use the following command:
nvme connect-all -t rdma -a $IPADDR
This requires the latest version of nvme-cli with Fabrics support.

The NVMe over Fabrics library provides an interface for both transports
and the nvme core to handle fabrics specific commands and attributes
independent of the underlying transport.
In addition, the fabrics library adds a misc device interface that allow
actually creating a fabrics controller, as we can't just autodiscover
it like in the PCI case. The nvme-cli utility has been enhanced to use
this interface to support fabric connect and discovery.

NVMe over Fabrics drivers are going to reuse the core,
so splits nvme.ko into 2 modules:
nvme-core.ko: the core part
nvme.ko: the PCI driver
Export symbols from nvme-core.ko.

Translation SCSI commands to NVMe commands is rather pointless in general
as applications must not expext to be able to use SCSI commands on a
generic block device.
Make the huge translation layer optional and hope no one will ever enable
it in the future.

The nvme driver was moved from drivers/block, losing our implicit
dependency on CONFIG_BLOCK. This makes it an explicit driver dependency.

This patch moves the NVMe driver from drivers/block/ to its own new
drivers/nvme/host/ directory. This is in preparation of splitting the
current monolithic driver up and add support for the upcoming NVMe
over Fabrics standard. The drivers/nvme/host/ is chose to leave space
for a NVMe target implementation in addition to this host side driver.
