Don't allow memory to be added above the allowed maximum allocation
limit set by Xen.

Scrubbing pages on initial balloon down can take some time, especially
in nested virtualization case (nested EPT is slow). When HVM/PVH guest is
started with memory= significantly lower than maxmem=, all the extra
pages will be scrubbed before returning to Xen. But since most of them
weren't used at all at that point, Xen needs to populate them first
(from populate-on-demand pool). In nested virt case (Xen inside KVM)
this slows down the guest boot by 15-30s with just 1.5GB needed to be
returned to Xen.
Add runtime parameter to enable/disable it, to allow initially disabling
scrubbing, then enable it back during boot (for example in initramfs).
Such usage relies on assumption that a) most pages ballooned out during
initial boot weren't used at all, and b) even if they were, very few
secrets are in the guest at that time (before any serious userspace
kicks in).
Convert CONFIG_XEN_SCRUB_PAGES to CONFIG_XEN_SCRUB_PAGES_DEFAULT (also
enabled by default), controlling default value for the new runtime
switch.

The current balloon code tries to calculate a delta factor for the
balloon target when running in HVM mode in order to account for memory
used by the firmware.
This workaround for memory accounting doesn't work properly on a PVH
Dom0, that has a static-max value different from the target value even
at startup. Note that this is not a problem for DomUs because guests are
started with a static-max value that matches the amount of RAM in the
memory map.
Fix this by forcefully setting target_diff for Dom0, regardless of
it's mode.

Commit 96edd61dcf44362d3ef0bed1a5361e0ac7886a63 ("xen/balloon: don't
online new memory initially") introduced a regression when booting a
HVM domain with memory less than mem-max: instead of ballooning down
immediately the system would try to use the memory up to mem-max
resulting in Xen crashing the domain.
For HVM domains the current size will be reflected in Xenstore node
memory/static-max instead of memory/target.
Additionally we have to trigger the ballooning process at once.

When setting up the Xenstore watch for the memory target size the new
watch will fire at once. Don't try to reach the configured target size
by onlining new memory in this case, as the current memory size will
be smaller in almost all cases due to e.g. BIOS reserved pages.
Onlining new memory will lead to more problems e.g. undesired conflicts
with NVMe devices meant to be operated as block devices.
Instead remember the difference between target size and current size
when the watch fires for the first time and apply it to any further
size changes, too.
In order to avoid races between balloon.c and xen-balloon.c init calls
do the xen-balloon.c initialization from balloon.c.

Today a Xenstore watch event is delivered via a callback function
declared as:
void (*callback)(struct xenbus_watch *,
const char **vec, unsigned int len);
As all watch events only ever come with two parameters (path and token)
changing the prototype to:
void (*callback)(struct xenbus_watch *,
const char *path, const char *token);
is the natural thing to do.
Apply this change and adapt all users.

Lets remove the modular code that is essentially orphaned, so that
when reading the driver there is no doubt it is builtin-only.
In doing so we uncover two implict includes that were obtained
by module.h having such a wide include scope itself:

Convert printks to pr_<level> (excludes printk(KERN_DEBUG...)
to be more consistent throughout the xen subsystem.
Add pr_fmt with KBUILD_MODNAME or "xen:" KBUILD_MODNAME
Coalesce formats and add missing word spaces

Align arguments and reflow to 80 columns
Remove DRV_NAME from formats as pr_fmt adds the same content
This does change some of the prefixes of these messages
but it also does make them more consistent.

Pull xen updates from Konrad Rzeszutek Wilk:

- PV multiconsole support, so that there can be hvc1, hvc2, etc; This
can be used in HVM and in PV mode.
- P-state and C-state power management driver that uploads said power
management data to the hypervisor. It also inhibits cpufreq

management decisions - fixing a weird perf bug.
There is one thing in the Kconfig that you won't like: "default y
if (X86_ACPI_CPUFREQ = y || X86_POWERNOW_K8 = y)" (note, that it
all depends on CONFIG_XEN which depends on CONFIG_PARAVIRT which by
default is off). I've a fix to convert that boolean expression
into "default m" which I am going to post after the cpufreq git
pull - as the two patches to make this work depend on a fix in Dave
Jones's tree.

"xen-balloon: convert sysdev_class to a regular subsystem" we would
end up with the attributes being put in:
/sys/devices/xen_memory0/target_kb
instead of

Making the tools inable to deflate the kernel to make more space
for launching another guest and printing:

This patch introduces two in-kernel drivers for Xen transcendent memory
("tmem") functionality that complement cleancache and frontswap. Both
use control theory to dynamically adjust and optimize memory utilization.
Selfballooning controls the in-kernel Xen balloon driver, targeting a goal
value (vm_committed_as), thus pushing less frequently used clean
page cache pages (through the cleancache code) into Xen tmem where
Xen can balance needs across all VMs residing on the physical machine.
Frontswap-selfshrinking controls the number of pages in frontswap,
driving it towards zero (effectively doing a partial swapoff) when
in-kernel memory pressure subsides, freeing up RAM for other VMs.

The basic functionality of ballooning pages is useful for Xen drivers in
general. Rather than require a dependency on the balloon module, split
the functionality that is reused into the core. The balloon module is
still required to follow ballooning requests from xenstore or to view
balloon statistics in sysfs.
