Workaround the SCSI layer's problematic WRITE SAME heuristics by
disabling WRITE SAME in the DM multipath device's queue_limits if an
underlying device disabled it.
The WRITE SAME heuristics, with both the original commit 5db44863b6eb
("[SCSI] sd: Implement support for WRITE SAME") and the updated commit
66c28f971 ("[SCSI] sd: Update WRITE SAME heuristics"), default to enabling
WRITE SAME(10) even without successfully determining it is supported.
After the first failed WRITE SAME the SCSI layer will disable WRITE SAME
for the device (by setting sdkp->device->no_write_same which results in
'max_write_same_sectors' in device's queue_limits to be set to 0).
When a device is stacked ontop of such a SCSI device any changes to that
SCSI device's queue_limits do not automatically propagate up the stack.
As such, a DM multipath device will not have its WRITE SAME support
disabled. This causes the block layer to continue to issue WRITE SAME
requests to the mpath device which causes paths to fail and (if mpath IO
isn't configured to queue when no paths are available) it will result in
actual IO errors to the upper layers.
This fix doesn't help configurations that have additional devices
stacked ontop of the mpath device (e.g. LVM created linear DM devices
ontop). A proper fix that restacks all the queue_limits from the bottom
of the device stack up will need to be explored if SCSI will continue to
use this model of optimistically allowing op codes and then disabling
them after they fail for the first time.

LVM2, since version 2.02.96, creates origin with zero size, then loads
the snapshot driver and then loads the origin. Consequently, the
snapshot driver sees the origin size zero and sets the hash size to the
lower bound 64. Such small hash table causes performance degradation.
This patch changes it so that the hash size is determined by the size of
snapshot volume, not minimum of origin and snapshot size. It doesn't
make sense to set the snapshot size significantly larger than the origin
size, so we do not need to take origin size into account when
calculating the hash size.

The kernel reports a lockdep warning if a snapshot is invalidated because
it runs out of space.
The lockdep warning was triggered by commit 0976dfc1d0cd80a4e9dfaf87bd87
("workqueue: Catch more locking problems with flush_work()") in v3.5.
The warning is false positive. The real cause for the warning is that
the lockdep engine treats different instances of md->lock as a single
lock.
This patch is a workaround - we use flush_workqueue instead of flush_work.
This code path is not performance sensitive (it is called only on
initialization or invalidation), thus it doesn't matter that we flush the
whole workqueue.
The real fix for the problem would be to teach the lockdep engine to treat
different instances of md->lock as separate locks.

There was a deliberate race condition in dm_stat_for_entry() to avoid the
overhead of disabling and enabling interrupts. The race could result in
some events not being counted on 64-bit architectures.
However, on 32-bit architectures, operations on long long variables are
not atomic, so the race condition could cause the counter to jump by 2^32.
Such jumps could be disruptive, so we need to do proper locking on 32-bit
architectures.

Since ENOSPC is a target-side error, dm-mpath should just pass the error
information to upper layer instead of retrying itself with path failover.
Otherwise it will end up failing all paths down while path checkers find
all paths ok.
ENOSPC can now be returned from SCSI device after commit a9d6ceb8
("[SCSI] return ENOSPC on thin provisioning failure").

The Armada XP timer has two mandatory clock inputs: nbclk and refclk,
as specified by the device-tree binding.
This commit fixes the clock selection. Instead of hard-coding the clock
rate for the 25 MHz reference fixed-clock, obtain the clock by its name.

The Armada XP SoC clocksource driver cannot work without the 25 MHz
fixed timer. Therefore it's appropriate to introduce a new compatible
string and use it to set the 25 MHz fixed timer.
The 'marvell,timer-25MHz' property will be marked as deprecated.

This commit creates two functions to access the TIMER_CTRL register:
one for global one for the per-cpu. This makes the code much more
readable. In addition, since the TIMER_CTRL register is also used for
watchdog, this is preparation work for future thread-safe improvements.

This change complements commit d0da7c002f7b2a93582187a9e3f73891a01d8ee4
and brings clear_ioasic_irq back, renaming it to clear_ioasic_dma_irq at
the same time, to make I/O ASIC DMA interrupts functional.
Unlike ordinary I/O ASIC interrupts DMA interrupts need to be deasserted
by software by writing 0 to the respective bit in I/O ASIC's System
Interrupt Register (SIR), similarly to how CP0.Cause.IP0 and CP0.Cause.IP1
bits are handled in the CPU (the difference is SIR DMA interrupt bits are
R/W0C so there's no need for an RMW cycle). Otherwise the handler is
reentered over and over again.
The only current user is the DEC LANCE Ethernet driver and its extremely
uncommon DMA memory error handler that does not care when exactly the
interrupt is cleared. Anticipating the use of DMA interrupts by the Zilog
SCC driver this change however exports clear_ioasic_dma_irq for device
drivers to choose the right application-specific sequence to clear the
request explicitly rather than calling it implicitly in the .irq_eoi
handler of `struct irq_chip'. Previously these interrupts were cleared in
the .end handler of the said structure, before it was removed.

Pull misc SCSI driver updates from James Bottomley:
"This patch set is a set of driver updates (megaraid_sas, fnic, lpfc,
ufs, hpsa) we also have a couple of bug fixes (sd out of bounds and
ibmvfc error handling) and the first round of esas2r checker fixes and
finally the much anticipated big endian additions for megaraid_sas"

Introduced module params to provide dynamic way of configuring
queue depth.
Added support to get max io throttle count through UCSM to
configure maximum outstanding IOs supported by fnic and push
that value to scsi mid-layer.
Supported IO throttle values:
UCSM IO THROTTLE VALUE FNIC MAX OUTSTANDING IOS

16 (Default) 2048

Kernel panics due to NULL lport while executing the log message because
of synchronization issues between libfc and scsi transport fc. Checking
for NULL pointers at the beginning of this routine would resolve the issue
from kernel panic point of view.

Hitting BUG_ON(io_req->abts_done) in fnic_rport_exch_reset in case of
timing issue and also to some extent locking issue where abts and terminate
is happening around same timing.
The code changes are intended to update CMD_STATE(sc) and
io_req->abts_done together.

Issue was seen when SCSI buffer address is more than 40 bits in system
with more than 1.1TB RAM. When SCSI buffer is passed to VIC, it is failing
to map to correct buffer address, as DMA mask is set to 40 bits in driver
initialization. Corrected DMA_MASK from 40-bits to 64-bits to avoid masking
41-64 bits addresses.

This patch fixes an out-of-bounds error in sd_read_cache_type(), found
by Google's AddressSanitizer tool. When the loop ends, we know that
"offset" lies beyond the end of the data in the buffer, so no Caching
mode page was found. In theory it may be present, but the buffer size
is limited to 512 bytes.
