When running as a Xen PV dom0 it can happen that the kernel is being
loaded to a guest physical address conflicting with the host memory
map.
In order to be able to resolve this conflict, add the capability to
remap non-RAM areas to different guest PFNs. A function to use this
remapping information for other purposes than doing the remap will be
added when needed.
As the number of conflicts should be rather low (currently only
machines with max. 1 conflict are known), save the remap data in a
small statically allocated array.

When booting as a Xen PV dom0 the memory layout of the dom0 is
modified to match that of the host, as this requires less changes in
the kernel for supporting Xen.
There are some cases, though, which are problematic, as it is the Xen
hypervisor selecting the kernel's load address plus some other data,
which might conflict with the host's memory map.
These conflicts are detected at boot time and result in a boot error.
In order to support handling at least some of these conflicts in
future, introduce a generic helper function which will later gain the
ability to adapt the memory layout when possible.
Add the missing check for the xen_start_info area.
Note that possible p2m map and initrd memory conflicts are handled
already by copying the data to memory areas not conflicting with the
memory map. The initial stack allocated by Xen doesn't need to be
checked, as early boot code is switching to the statically allocated
initial kernel stack. Initial page tables and the kernel itself will
be handled later.

The recent adding of multicall debug mixed up the referencing of

Under arch/x86/xen there is one large private header file xen-ops.h
containing most of the Xen-private x86 related declarations, and then
there are several small headers with a handful of declarations each.
Merge the small headers into xen-ops.h.
While doing that, move the declaration of xen_fifo_events from
xen-ops.h into include/xen/events.h where it should have been from the
beginning.

When running as PVH or HVM Linux will use holes in the memory map as scratch
space to map grants, foreign domain pages and possibly miscellaneous other
stuff. However the usage of such memory map holes for Xen purposes can be
problematic. The request of holesby Xen happen quite early in the kernel boot
process (grant table setup already uses scratch map space), and it's possible
that by then not all devices have reclaimed their MMIO space. It's not
unlikely for chunks of Xen scratch map space to end up using PCI bridge MMIO
window memory, which (as expected) causes quite a lot of issues in the system.
At least for PVH dom0 we have the possibility of using regions marked as
UNUSABLE in the e820 memory map. Either if the region is UNUSABLE in the
native memory map, or it has been converted into UNUSABLE in order to hide RAM
regions from dom0, the second stage translation page-tables can populate those
areas without issues.
PV already has this kind of logic, where the balloon driver is inflated at
boot. Re-use the current logic in order to also inflate it when running as
PVH. onvert UNUSABLE regions up to the ratio specified in EXTRA_MEM_RATIO to
RAM, while reserving them using xen_add_extra_mem() (which is also moved so
it's no longer tied to CONFIG_PV).

Today the percpu struct vcpu_info is allocated via DEFINE_PER_CPU(),
meaning that it could cross a page boundary. In this case registering
it with the hypervisor will fail, resulting in a panic().
This can easily be fixed by using DEFINE_PER_CPU_ALIGNED() instead,
as struct vcpu_info is guaranteed to have a size of 64 bytes, matching
the cache line size of x86 64-bit processors (Xen doesn't support
32-bit processors).

The flag xen_have_vcpu_info_placement was needed to support Xen
hypervisors older than version 3.4, which didn't support the
VCPUOP_register_vcpu_info hypercall. Today the Linux kernel requires
at least Xen 4.0 to be able to run, so xen_have_vcpu_info_placement
can be dropped (in theory the flag was used to ensure a working kernel
even in case of the VCPUOP_register_vcpu_info hypercall failing for
other reasons than the hypercall not being supported, but the only
cases covered by the flag would be parameter errors, which ought not
to be made anyway).
This allows to let some functions return void now, as they can never
fail.

This was effectively lost while dropping PVHv1 code. Move the function
and arrange for it to be called the same way as done in PV mode. Clearly
this then needs re-introducing the XENFEAT_mmu_pt_update_preserve_ad

Since the string pointed at by pv_info.name describes the mode, drop
"paravirtualized" from the log message while moving the code.

Without announcing hvc0 as preferred it won't get used as long as tty0
gets registered earlier. This is particularly problematic with there not
being any screen output for PVH Dom0 when the screen is in graphics
mode, as the necessary information doesn't get conveyed yet from the
hypervisor.
Follow PV's model, but be conservative and do this for Dom0 only for
now.

Decouple XEN_DOM0 from XEN_PV, converting some existing uses of XEN_DOM0
to a new XEN_PV_DOM0. (I'm not convinced all are really / should really
be PV-specific, but for starters I've tried to be conservative.)
For PVH Dom0 the hypervisor populates MADT with only x2APIC entries, so
without x2APIC support enabled in the kernel things aren't going to work
very well. (As opposed, DomU-s would only ever see LAPIC entries in MADT
as of now.) Note that this then requires PVH Dom0 to be 64-bit, as
X86_X2APIC depends on X86_64.
In the course of this xen_running_on_version_or_later() needs to be
available more broadly. Move it from a PV-specific to a generic file,
considering that what it does isn't really PV-specific at all anyway.
Note that xen/interface/version.h cannot be included on its own; in
enlighten.c, which uses SCHEDOP_* anyway, include xen/interface/sched.h
first to resolve the apparently sole missing type (xen_ulong_t).

USERGS_SYSRET64 is used to return from a syscall via SYSRET, but
a Xen PV guest will nevertheless use the IRET hypercall, as there
is no sysret PV hypercall defined.
So instead of testing all the prerequisites for doing a sysret and
then mangling the stack for Xen PV again for doing an iret just use
the iret exit from the beginning.
This can easily be done via an ALTERNATIVE like it is done for the
sysenter compat case already.
It should be noted that this drops the optimization in Xen for not
restoring a few registers when returning to user mode, but it seems
as if the saved instructions in the kernel more than compensate for
this drop (a kernel build in a Xen PV guest was slightly faster with
this patch applied).
While at it remove the stale sysret32 remnants.

Kbuild test robot reports the following problem on ARM:
for 'xen_setup_callback_vector' [-Wmissing-prototypes]
1664 | void xen_setup_callback_vector(void) {}
| ^~~~~~~~~~~~~~~~~~~~~~~~~
The problem is that xen_setup_callback_vector is a x86 only thing, its
definition is present in arch/x86/xen/xen-ops.h but not on ARM. In
events_base.c there is a stub for !CONFIG_XEN_PVHVM but it is not declared
as 'static'.
On x86 the situation is hardly better: drivers/xen/events/events_base.c
doesn't include 'xen-ops.h' from arch/x86/xen/, it includes its namesake
from include/xen/ which also results in a 'no previous prototype' warning.
Currently, xen_setup_callback_vector() has two call sites: one in
drivers/xen/events_base.c and another in arch/x86/xen/suspend_hvm.c. The
former is placed under #ifdef CONFIG_X86 and the later is only compiled
in when CONFIG_XEN_PVHVM.
Resolve the issue by moving xen_setup_callback_vector() declaration to
arch neutral 'include/xen/hvm.h' as the implementation lives in arch
neutral drivers/xen/events/events_base.c.

Convert the XEN/PV hypercall to IDTENTRY:
- Emit the ASM stub with DECLARE_IDTENTRY
- Remove the ASM idtentry in 64-bit
- Remove the open coded ASM entry code in 32-bit
- Remove the old prototypes
The handler stubs need to stay in ASM code as they need corner case handling
and adjustment of the stack pointer.
Provide a new C function which invokes the entry/exit handling and calls
into the XEN handler on the interrupt stack if required.
The exit code is slightly different from the regular idtentry_exit() on
non-preemptible kernels. If the hypercall is preemptible and need_resched()
is set then XEN provides a preempt hypercall scheduling function.
Move this functionality into the entry code so it can use the existing
idtentry functionality.

In every hypervisor except for xen pv time ops are initialized in
init_hypervisor_platform().
Xen PV domains initialize time ops in x86_init.paging.pagetable_init(),
by calling xen_setup_shared_info() which is a poor design, as time is
needed prior to memory allocator.
xen_setup_shared_info() is called from two places: during boot, and
after suspend. Split the content of xen_setup_shared_info() into
three places:
1. add the clock relavent data into new xen pv init_platform vector, and
set clock ops in there.
2. move xen_setup_vcpu_info_placement() to new xen_pv_guest_late_init()

In order to support pvclock vdso on xen we need to setup the time
info page for vcpu 0 and register the page with Xen using the
VCPUOP_register_vcpu_time_memory_area hypercall. This hypercall
will also forcefully update the pvti which will set some of the
necessary flags for vdso. Afterwards we check if it supports the
PVCLOCK_TSC_STABLE_BIT flag which is mandatory for having
vdso/vsyscall support. And if so, it will set the cpu 0 pvti that
will be later on used when mapping the vdso image.
The xen headers are also updated to include the new hypercall for
registering the secondary vcpu_time_info struct.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:
SPDX license identifier # files
---------------------------------------------------|-------
GPL-2.0 11139
and resulted in the first patch in this series.
If that file was a */uapi/* path one, it was "GPL-2.0 WITH
Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was:
SPDX license identifier # files
---------------------------------------------------|-------
GPL-2.0 WITH Linux-syscall-note 930
and resulted in the second patch in this series.
- if a file had some form of licensing information in it, and was one
of the */uapi/* ones, it was denoted with the Linux-syscall-note if
any GPL family license was found in the file or had no licensing in
it (per prior point). Results summary:
SPDX license identifier # files
---------------------------------------------------|------
GPL-2.0 WITH Linux-syscall-note 270
GPL-2.0+ WITH Linux-syscall-note 169
((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause) 21
((GPL-2.0 WITH Linux-syscall-note) OR BSD-3-Clause) 17
LGPL-2.1+ WITH Linux-syscall-note 15
GPL-1.0+ WITH Linux-syscall-note 14
((GPL-2.0+ WITH Linux-syscall-note) OR BSD-3-Clause) 5
LGPL-2.0+ WITH Linux-syscall-note 4
LGPL-2.1 WITH Linux-syscall-note 3
((GPL-2.0 WITH Linux-syscall-note) OR MIT) 3
((GPL-2.0 WITH Linux-syscall-note) AND MIT) 1
and that resulted in the third patch in this series.
- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

When running as Xen pv-guest the exception frame on the stack contains
%r11 and %rcx additional to the other data pushed by the processor.
Instead of having a paravirt op being called for each exception type
prepend the Xen specific code to each exception entry. When running as
Xen pv-guest just use the exception entry with prepended instructions,
otherwise use the entry without the Xen specific code.

Xen's paravirt patch function xen_patch() does some special casing for
irq_ops functions to apply relocations when those functions can be
patched inline instead of calls.
Unfortunately none of the special case function replacements is small
enough to be patched inline, so the special case never applies.
As xen_patch() will call paravirt_patch_default() in all cases it can
be just dropped. xen-asm.h doesn't seem necessary without xen_patch()
as the only thing left in it would be the definition of XEN_EFLAGS_NMI
used only once. So move that definition and remove xen-asm.h.

The hypercall VCPUOP_register_vcpu_info can fail. This failure is
handled by making per_cpu(xen_vcpu, cpu) point to its shared_info
slot and those without one (cpu >= MAX_VIRT_CPUS) be NULL.
For PVH/PVHVM, this is not enough, because we also need to pull
these VCPUs out of circulation.
Fix for PVH/PVHVM: on registration failure in the cpuhp prepare
callback (xen_cpu_up_prepare_hvm()), return an error to the cpuhp
state-machine so it can fail the CPU init.
Fix for PV: the registration happens before smp_init(), so, in the
failure case we clamp setup_max_cpus and limit the number of VCPUs
that smp_init() will bring-up to MAX_VIRT_CPUS.
This is functionally correct but it makes the code a bit simpler
if we get rid of this explicit clamping: for VCPUs that don't have
valid xen_vcpu, fail the CPU init in the cpuhp prepare callback
(xen_cpu_up_prepare_pv()).

Historically we didn't call VCPUOP_register_vcpu_info for CPU0 for
PVHVM guests (while we had it for PV and ARM guests). This is usually
fine as we can use vcpu info in the shared_info page but when we try
booting on a vCPU with Xen's vCPU id > 31 (e.g. when we try to kdump
after crashing on this CPU) we're not able to boot.

Check whether the hypervisor supplied p2m list is placed at a location
which is conflicting with the target E820 map. If this is the case
relocate it to a new area unused up to now and compliant to the E820
map.
As the p2m list might by huge (up to several GB) and is required to be
mapped virtually, set up a temporary mapping for the copied list.
For pvh domains just delete the p2m related information from start
info instead of reserving the p2m memory, as we don't need it at all.
For 32 bit kernels adjust the memblock_reserve() parameters in order
to cover the page tables only. This requires to memblock_reserve() the
start_info page on it's own.

Some special pages containing interfaces to xen are being reserved
implicitly only today. The memblock_reserve() call to reserve them is
meant to reserve the p2m list supplied by xen. It is just reserving
not only the p2m list itself, but some more pages up to the start of
the xen built page tables.
To be able to move the p2m list to another pfn range, which is needed
for support of huge RAM, this memblock_reserve() must be split up to
cover all affected reserved pages explicitly.
The affected pages are:
- start_info page
- xenstore ring (might be missing, mfn is 0 in this case)
- console ring (not for initial domain)

It turns out that a PV domU also requires the "Xen PV" APIC
driver. Otherwise, the flat driver is used and we get stuck in busy
loops that never exit, such as in this stack trace:
