Resolves several issues in UefiCpuPkg related to:
1. Unchecked returns leading to potential NULL or uninitialized access.
2. Potential unchecked integer overflows.
3. Incorrect comparison between integers of different sizes.

The purpose of writing CR3 in ConvertMemoryPageToNotPresent is just
to flush TLB, because CR3 won't be changed in function
ConvertMemoryPageToNotPresent.
After ConvertMemoryPageToNotPresent, there is always a flush TLB
function. Also, because ConvertMemoryPageToNotPresent in called in a
loop, to improve performance, there is no need to flush TLB
inside ConvertMemoryPageToNotPresent. Just flushing TLB after the loop
is enough.

Currently, there are code to set memory attribute in CpuMpPei module.
However, the code doesn't handle the case of 5 level paging.
Use the CpuPageTableLib to set memory attribute for two purpose:
1. Add 5 level paging support
2. Clean up code

Today InitializeCpuExceptionHandlersEx is called from three modules:
1. DxeCore (links to DxeCpuExceptionHandlerLib)
DxeCore expects it initializes the IDT entries as well as
assigning separate stacks for #DF and #PF.
2. CpuMpPei (links to PeiCpuExceptionHandlerLib)
and CpuDxe (links to DxeCpuExceptionHandlerLib)
It's called for each thread for only assigning separate stacks for
#DF and #PF. The IDT entries initialization is skipped because
caller sets InitData->X64.InitDefaultHandlers to FALSE.
Additionally, SecPeiCpuExceptionHandlerLib, SmmCpuExceptionHandlerLib
also implement such API and the behavior of the API is simply to initialize
IDT entries only.
Because it mixes the IDT entries initialization and separate stacks
assignment for certain exception handlers together, in order to know
whether the function call only initializes IDT entries, or assigns stacks,
we need to check:
1. value of InitData->X64.InitDefaultHandlers
2. library instance
This patch cleans up the code to separate the stack assignment to a new API:
InitializeSeparateExceptionStacks().
Only when caller calls the new API, the separate stacks are assigned.
With this change, the SecPei and Smm instance can return unsupported which
gives caller a very clear status.
The old API InitializeCpuExceptionHandlersEx() is removed in this patch.
Because no platform module is consuming the old API, the impact is none.

When StackGuard is enabled, the CpuMp driver allocates
known good stacks for all CPUs for DF# and PF# exceptions.
It uses AllocatePool to do so.
The size needed equals to 64KB
= StackSize (2K) * ExceptionNumber (2) * NumberOfProcessors (16)
However, AllocatePool max allocation size is less than 64K.
To fix the issue, AllocatePages() is used.

This feature is the same as Stack Guard enabled in driver CpuDxe but
applies to PEI phase. Due to the specialty in PEI module dispatching,
this driver is changed to do the actual initialization in notify
callback of event gEfiPeiMemoryDiscoveredPpiGuid. This can let the
stack guard apply to as most PEI drivers as possible.
To let Stack Guard work, some simple page table management code are
introduced to setup Guard page at base of stack for each processor.

Using CodeSegment and DataSegment fields in ExchangeInfo instead of the hardcode
MACROs for x64 arch. Switch AP from real mode to long mode directly, so needn't

Verification microcode signature is one enhancement and not one requirement from
IA32 SDM. This update is just to dump debug message instead of ASSERT() if the
updated microcode signature does not match the loaded microcode signature.

Actually, there is only one microcode region in platform. If microcode has been
loaded, its signature will not be zero and should be loaded successfully.
We needn't to check microcode region and load microcode again. This update is to
skip checking/loading microcode if current microcode signature is not zero.

If ApLoopMode is set to ApInMwaitLoop, AP will be placed into C-State by mwait
instruction. BSP will wakeup AP by write start-up signal in monitor address.
However, AP maybe waken by SMI/NMI/MCE and other condition. On this case, AP
will check if BSP wants to wakeup itself really. If not, AP will continue to
execute mwait to C-State.
One potential issue: BSP may not recognize AP was wakeup from C-State by other
event and BSP still writes start-up signal to wakeup AP. But AP does not aware
it and still execute mwait instruction to C-State. So, AP cannot be wakeup on
this case.
This fix is let AP to clear start-up signal when it really is wakeup to execute
AP function. And BSP will write start-up signal till AP clears it.
