As with the previous patch EEH is always enabled if SPAPR_TCE_IOMMU, so
move this last bit of code into the main module.
Now that this function only processes VFIO_EEH_PE_OP remove a level of
indenting as well, it is only called by a case statement that already
checked VFIO_EEH_PE_OP.
This eliminates an unnecessary module and SPAPR code in a global header.

The PPC64 kconfig is a bit of a rats nest, but it turns out that if
CONFIG_SPAPR_TCE_IOMMU is on then EEH must be too:

The vfio_spapr_pci_eeh_open/release() functions are one line wrappers
around an arch function. Just call them directly. This eliminates some
weird exported symbols that don't need to exist.

Currently, the EEH recovery process considers passed-through devices
as if they were not EEH-aware, which can cause them to be removed as
part of recovery. Because device removal requires cooperation from
the guest, this may lead to the process stalling or deadlocking.
Also, if devices are removed on the host side, they will be removed
from their IOMMU group, making recovery in the guest impossible.
Therefore, alter the recovery process so that passed-through devices
are not removed but are instead left frozen (and marked isolated)
until the guest performs it's own recovery. If firmware thaws a
passed-through PE because it's parent PE has been thawed (because it
was not passed through), re-freeze it.

The patch adds one more EEH sub-command (VFIO_EEH_PE_INJECT_ERR)
to inject the specified EEH error, which is represented by
(struct vfio_eeh_pe_err), to the indicated PE for testing purpose.

The existing vfio_pci_open() fails upon error returned from
vfio_spapr_pci_eeh_open(), which breaks POWER7's P5IOC2 PHB
support which this patch brings back.
The patch fixes the issue by dropping the return value of
vfio_spapr_pci_eeh_open().

The VFIO related components could be built as dynamic modules.
Unfortunately, CONFIG_EEH can't be configured to "m". The patch
fixes the build errors when configuring VFIO related components
as dynamic modules as follows:
